LAB SAFETY ORIENTATION CHECKLIST
Attached is a lab safety orientation checklist tool that can be used to communicate critical safety and environmental information to the people working in your lab.
Instructions
Complete the sections below with information specific to your lab. Information should include important telephone numbers, evacuation routes and assembly points. Indicate the location of safety and environmental equipment (ex. on east wall or near the sink). Include locations of alternate equipment if there is a risk that the lab will need to be evacuated. Delete all non-applicable items.
Suggested Uses
Provide a copy of the checklist to all new lab personnel. Have them locate each piece of safety and environmental equipment and sign the statement at the end of the checklist.
Have lab personnel draw a map of the lab indicating the location of each piece of safety and environmental equipment.
Review the checklist periodically during meetings with lab personnel.
Post this checklist in the lab along with a room layout showing the location of emergency equipment and evacuation routes. (The Environmental Health and Safety (EHS) Office can laminate the checklist and room layout upon request.)
