to furnish all the necessary labor, equipment and material to perform all work in accordance with the projects scope-of-work, State of Vermont 2011 Standard Specifications for Construction with all general special provisions, Project Documents, and the description of construction as contained herein, for full width paving of the listed project(s); on which sealed or electronic bids will be received until 10:00AM prevailing time Thursday April 30, 2015. Sealed bids will be received at the Cedarville Town Office located at 4321 Center Rd. Cedarville, VT 55555-5555. Electronic bids will be received by e-mail at the Cedarville Town Office by Velma Pearson at . The time of receiving and opening of submitted bids may be postponed by the Town of Cedarville due to emergencies and unforeseen circumstances and/or conditions. The projects are situated as follows:
Project Locations: (1) Elm St. (TH 36) - at Hardware Store @ MM 0.20 to Billet Farm Rd. @ MM 1.30 at 25 feet wide and 1.10 miles long; (2) Maple Hill Rd. (TH 4) – by school @ MM 0.35 to recycle center @ MM 2.77 at 27.5 feet wide and 2.42 miles long.
Construction Being: Cold planing beginning and end project tapers, cold planing bridges and begin and end tapers to bridges, cold planing shoulder and side road tapers, cold planing longitudinal tapers, clean and prepare road surface for application of emulsion and pavement, pre-shim designated areas, tack the cleaned and prepped surface to be paved, placement and compaction of bituminous concrete pavement, traffic control to include flaggers and all appropriate signs, the clean up and removal of waste bituminous concrete pavement, and the application of line striping targets. Construction of each individual project may be different and each project may not require all items listed above. Project specific construction details shall be included in each project’s Scope-of-Work.

Certification of Bid Proposal: In accordance with the projects Scope-of-Work, Check List, State of Vermont 2011 Standard Specifications for Construction with all general special provisions and attachments, Project Documents, and the description of construction as shown above, I, or we, hereby certify that I am, or we are, the only person or persons interested in this Proposal as principal or principals; that it is made without collusion with any person, firm, or corporation; that an examination has been made of the Proposal, the provided documents, the Specifications and the site of the work, and that I, or we, propose to furnish all necessary machinery, equipment, tools, labor and other means of construction, and all materials specified, in the manner and at the time prescribed, understanding that the quantities of work as shown herein are only estimated and approximate and are subject to increase or decrease and that all quantities of work, whether increased or decreased, are to be performed at the following unit prices:

CONSIDERATION OF BIDS:
Bids not submitted on the bid form provided will not be accepted. Incomplete bids may not be accepted. Each Project will be awarded separately. Bids, if awarded, will be awarded based on the following criteria:
- Least cost per ton in place or least total project cost for projects with multiple bid items.
- Towns to add any criteria they see germane.
NOTES:
1. The Town reserves the right to reject any and all bids in part or in whole.
2. The Town reserves the right to cancel any project without prior notice.
3. All work shall be performed in accordance with the contract documents and the most recent version of the Vermont Agency of Transportation Standard Specifications for Construction with all amendments and general special conditions.
4. All communications will go through the Town of Cedarville, Town Clerk, Nancy Button. Any question(s) posed during the bidding process will be distributed electronically to all bidders in a question – answer format.
5. The question period will end 5:00PM Thursday April 23, 2015. Any questions posed after the date listed above shall go unanswered.
6. If multiple bids are submitted, the last bid submitted prior to the submitted by date and time will be considered the official bid. All previously submitted bids will not be opened and will be discarded.
7. All bidders must supply a list of Authorized Representatives to submit bids electronically on their behalf at the Pre-Bid Conference or 48 hours prior to bid due date and time to the Town of Cedarville Town Clerk, Nancy Button.
8. All item quantities are estimates and the Town does not guarantee any or all quantities estimated will be used.
9. The Contractor must supply copies of a State approved mix designs prior to beginning of construction.
10. The Plant producing the bituminous concrete pavement shall have a current year certification by the State of Vermont prior to producing mix for the specified project(s).
11. The Contractor shall provide notification 24 hours in advance requesting to attain Bituminous Concrete Pavement from a different plant after construction has commenced.
12. All bids for Bituminous Concrete Pavement will be for a TYPE IV – 50 Blow for Project (1). Project (2) specifies both a TYPE II and TYPE III – 75 Blow.
13. The contractor shall provide 72 hours notice to the Town of Cedarville Town Clerk, Nancy Button, prior to the start of work.
14. All RAP generated by the project is the property of the Town of Cedarville. The RAP is to be delivered to the Town Maintenance garage located at 789 Highway Drive Cedarville, VT at the submitted bid price.
15. Emulsion will be a RS-1 or RS-1H and will be applied to all surfaces except gravel being paved at a rate of 0.050 gal/SY.

17. The cleaning of haul units and paving equipment on the surface to be paved shall not be allowed. The contractor is responsible for locating on or off project(s) sites for this activity. All waste material generated from this activity shall be removed from the project(s) within 48 hours upon completion of the project at the sole expense of the contractor.
18. The contractor is expected to work within the Town right-of-way. Any cost associated with the contractor’s desire to obtain clearances to work outside the existing right-of-way will be at the sole expense of the contractor.
19. Damage by the contractor to Town and/or personnel property, or Utilities during construction will be repaired to pre-construction condition and will be at the sole expense of the contractor.
20. The contractor is required to provide traffic control for the duration of the project(s).
21. The project(s) completion date is September 18, 2015. No paving will be done after Saturday September 26, 2016 without written consent from the Town of Cedarville.
