Your college may already require you to keep an ongoing log of your externship/practicum experience. We strongly encourage you to keep track of the following activities:

1.  A calendar of the days you spent on-center, as well as the time/hours

2.  Supervision hours (including date, duration)

3.  Client contact hours (especially for graduate students)

4.  Types of activities (this will come in handy for the final evaluation & future resume)

A sample log follows this page. You can use this log, although your college may require you to use a specific form instead.
