Re: Conversion from ICD 9 Codes to ICD 10 Codes and WHODAS requirement
Beginning October 1, 2015, the Minnesota Department of Health requires that all mental health providers transition from using ICD 9 diagnostic criteria and move to using ICD 10 codes. In order to prevent any lapses in services for clients we are asking that you include both the ICD 9 and ICD 10 codes on the Diagnostic Assessment form beginning immediately. For more information regarding the ICD conversion please refer to the MDH ICD 10 website.
Please also note that the WHODAS is required as part of a diagnostic assessment (DA). The WHODAS replaces axis V on the former DSM axial system and must be available for auditing purposes. You can find the WHODAS forms and more information on the Department of Human Services WHODAS website.
Attached is an updated DA form which Hennepin County Human Services and Public Health provides clinicians to determine eligibility for Adult Mental Health Case Management, Intensive Residential Treatment Services, ARMHS and related community mental health services. The DA form is provided for your convenience and helps expedite obtaining mental health case management services for persons with serious and persistent mental illness to gain access to medical, psychosocial, vocational and other necessary services to assist in their recovery process. If your agency or practice utilizes an alternative format for a DA, it should include all the elements of the Hennepin County form or otherwise meet the criteria for the standard diagnostic assessment as prescribed in Chapter 16 of the MHCP Provider Manual.
