Paw Sox Permission / Information Form
Dear Parent/Guardian:
The Talbot Innovation Middle School Marching Band will be performing at the Pawtucket Red Sox on Tuesday, June 9, 2015. The students will perform a short pre-game program between 6:00 – 7:00 p.m. and then take the field again to perform the National Anthem prior to the 7:05 p.m. game. We encourage family and friends to attend this important Band fundraiser. General admission tickets may be ordered for $9.00. A portion of the ticket price will be returned to the Band.
* All ticket sales are final. In the event of inclement weather, tickets may be used for the next home game.
Band and Color Guard students will attend the game free of charge. Band students are required to sell at least one game ticket. Bus transportation will be provided for all participating students. Food is available for purchase at the park.
Report to Talbot: 4:00 p.m. (door #6)
Depart McCoy Stadium: 8:45 p.m.
Return to Talbot: 9:30 p.m. (door # 6)
Dress: Black Band Shirts and Black pants or shorts

------I give permission for my child______to attend the Talbot Innovation Middle School Band performance at the Pawtucket Red Sox game on Tuesday, June 9, 2015. I understand that my child will be performing at the game and may be photographed or video taped.
______My child will return to Talbot Middle School on the bus with Ms. Souza
______I will provide transportation home for my child.
_____My child is in 8th Grade. Please release him/her to the Grade 8 field trip after the performance.
If you would like your child to sit with you during the game you may sign them out of the field trip after the performance.
