TERMS FOR FACILITY USE
1. This agreement is for the specified room(s) and time only(s). Building facilities must be vacated by the time specified on the Agreement. Failure to comply will result in additional fees.
2. Insurance. In compliance with Church Mutual Insurance Company, an outside organization using these premises will be required to furnish a certificate of insurance as proof that they have adequate liability and medical expense coverage. We will request that University UMC and/or Wesley Foundation at Michigan State University be named as an "additional insured" on their policy. Proof of Insurance must be received one week prior to the event.
3. General Policies. Based on religious convictions and beliefs, there is to be no smoking, alcohol use, or illegal drug use allowed anywhere on the premises at any time. Raffles, bingo games, and/or any other forms of gambling are not allowed in or on the premises. No one may sell tickets for or conduct a drawing connected with such activity on church property. Pets (except service dogs) are not allowed anywhere on the premises at any time.
4. Children. If this event includes attendance by children, or separate activities for children, the following applies:
a. Children are not to be separated from the main group unless supervised by a minimum of two adults at all times.
b. A minimum of two adults must be provided if childcare is being provided as part of this event. It is preferred that these adults have undergone a background check which is the responsibility of the organization using the facility.
c. Children are not to be left unsupervised at any time, nor are they to be allowed to enter any room(s) other than those specified for this activity.
d. If children play outside the building, supervision is required by adults utilizing the criteria above.
e. For the safety of children, they will not be permitted in the hallways without an adult present.
5. Room Set-up Requirements. Each room is furnished in a standard format and furnishings Furniture arrangements that require substantial modification of these formats may incur additional charges at a rate of $30 for the first hour and $15/hr thereafter. Once room use is contracted, the furniture arrangement for the room will be negotiated based on seating requirements and resources needed. Negotiated furniture arrangements may be modified prior to the actual set-up. Once room set-ups are in place, modification of the negotiated set-up will require approval of the facilities administrator or his representative. Additional fees may apply. Some audio visual equipment is available for a fee (see #8 below). Prior arrangements need to be made with the Facilities Administrator.
6. Room Capacity and Duration of Occupancy. The fees are based on attendance of up to 100 persons. More than 100 attendees will be billed at a negotiated higher rate based on the number of attendees. Fees are based on 5 hours of use. Overruns of allocated use are limited to 2 hours and the additional hours will be billed at 20% of the fee for each hour of use. A building host will be required for the duration of occupancy at an additional fee
7. Beverage and Food Service. Users may take advantage of the facilities for making coffee and tea in the Copper Chimney Lounge and in Asbury Hall at no additional charge. We have available for your use a Bunn Coffeemaker adjacent to these rooms. You must bring your own coffee and supplies. Coffee/beverage and food service is not provided by UUMC or Wesley Foundation staff. For food service there are kitchen facilities are available for rent in Asbury Hall. Food can be prepared by the user only for event attendees. Also, food service is available for a negotiated price through the kitchen committee.
8. AV Equipment, Sound System, & Piano. Some audio visual equipment is available for your use for a fee The building sound system can only be operated by a qualified sound technician. If used, a separate fee will be applied. There is a piano available for use at no cost; however, if piano tuning is requested, that will be charged at cost and paid for by the user. Prior arrangements for use of any equipment needs to be made with the Facilities Administrator.
9. Liability. I agree that the Wesley Foundation at Michigan State University and/or University UMC will not be held liable in the event of an accident or injury to any attendees resulting from participation in the described activity. The user, is responsible for breakage and damage of the facilities or property during the time the building facilities are entrusted to their care. Any breakage shall be reported immediately to the custodian or the Facilities Administrator on the day of business following your event. Wesley Foundation at Michigan State University or University UMC shall in no way be responsible for the loss theft, damage, or destruction of any materials, equipment, property, etc., brought onto the premises or parking lot.
10. Closures Due to Weather and Other Emergencies. If the church is closed, the event that was scheduled during the closure will be cancelled. In the event of closure before the start of an event, all fees will be refunded. Closure will be based on the schools or the city being closed. In addition, pastors will have the discretion to order closure or make exceptions.
