All prospective bidders are hereby advised of the following amendments:
I.OMBUDSMAN BID FORM NO. 1– TECHNICAL PROPOSAL
- Delivery period of “7 calendar days from receipt of Purchase Order”is amended to be “Thirty (30)calendar days from receipt of Purchase Order.”
- The color is added to the technical specifications: “Color: White”
II.CHECKLIST OF DOCUMENTS
With regards to Item No. I.A.1.b General Information Sheet for CY 2009 (Original or Certified True Copy by the SEC), bidders may also submit their“General Information Sheet for CY 2010(Original or Certified True Copy by the SEC).”
Bidders are advised to use attached the AMENDED OMBUDSMAN BID FORM NO. 1and AMENDED CHECKLIST OF DOCUMENTSin lieu of the previously issued bid forms.
