Note: It is difficult to miss any rehearsals as we work at an extremely fast pace—we will excuse up to 3 absences. Attendance at the last week of Technical and Dress rehearsals is required!

Thank you very much for your interest in “ShowTime”….Please keep in mind that we have more than 1 audition date and that our spaces are limited due to the number of costumes and the sizes of costumes that are available or in stock.

For women, the sizes range from 8 – 16 or small to XL. For men, the sizes range from medium to XXXL.

You will receive an e-mail, or letter indicating acceptance or other offer for participation as part of “ShowTime.”

Informed Consent For Participation In ShowTime

I, ______agree to attend rehearsals, to practice the dances, and to learn the music.

I understand that the cast and crew of ShowTime are volunteers and that participation in ShowTime requires an individual and team effort. I agree to do my best to work with the cast, crew, and directors.

Most of the costumes for ShowTime are provided; however, you will be expected to furnish a few items (undergarments, occasionally a shirt or blouse, or other costume item) and your own shoes as chosen by the directors. I agree to purchase the items that I need for participation in ShowTime.

I understand the physically active nature of performing as a participant in ShowTime. I understand that this program may or may not benefit my physical or general health. I also understand that there is a risk of injury from sprains/strains to heart attacks and possible death as a result of my exercise, but knowing these risks, I desire to participate.

I further understand that there are also other remote risks that may be associated with the activities related to this program and/or its instruction. Despite the fact that a complete accounting of these remote risks has not been provided to me, I still desire to participate.

If I have not regularly engaged in a physical activity recently, it will be in my best health interest to first consult with my physician before beginning this dance-based series of rehearsals and performances. I accept full responsibility for decreasing or stopping exercise if I experience any unusual pain or physical discomfort during participation in ShowTime.

I am aware that to be effective and safe, dance and other related exercises should be controlled by the participant; that is, the individual should work at the level of intensity appropriate for his or her physical condition.

I acknowledge that I have read this document in its entirety, or had it read to me if I am unable to read it, and desire to participate in this program provided by ShowTime as indicated by my signature on this form.
