Family Continuity

Family Continuity has Outpatient Clinics in Whitinsville and Worcester. The homebased team provides Children’s Behavioral Health Initiative services to include In Home Therapy, and Therapeutic Mentoring. The Community Support Program serves the Greater Worcester area and Southern Worcester County. Our Northbridge Coalition is Grassroots group with focus on critical community needs and strengths while our Blackstone Valley Connector partners with 5 localPolice Departments in providing post response calls and outreach. We service the Northbridge, Uxbridge, Sutton, Blackstone, and Millville areas. The Community Outreach Initiative Network (COIN) Innovative Jail Diversion Program supports a 10 town area in Northeastern Worcester and Northern Middlesex County to include Ayer, Shirley, and Ashby. Multi- Cultural Wellness Center

The Multicultural Wellness Center (MWC) is a non-profit mental health clinic. The mission of MWC is to provide mental health services to diverse communities who have historically been underserved and underrepresented in the mental health field.

MYR – Motivating Youth Recovery (Community Health Link)

MYR is a 24-hour inpatient program for adolescents in Massachusetts, between the ages of 13 and 17, who are working on recovery from addiction to drugs, alcohol, and/or inhalants. With approval, 18 year olds may be admitted on a case by case basis. MYR provides a safe environment for clients to detoxify, attain stability, address substance abuse and mental health issues and move on to the next level of care. MYR uses a multifaceted approach to recovery which includes medical detoxification, milieu treatment, a modified 12-step model, motivational interviewing techniques, art, music and cognitive behavioral therapy. This model promotes skill building, personal empowerment, and accountability. The program encourages family involvement and promotes the development of a strong network of peer and social support which will aid the client in sustaining long term recovery. MYR provides treatment and staffing 24 hours a day, seven days a week. While at MYR the adolescent will take part in a structured daily schedule. Treatment includes individual, family, and group psychotherapy. All clients will get an individual counselor who will guide them through the treatment process. All clients will also get a personal case manager, who will contact all collaterals and assist in setting up a personalized, effective aftercare and discharge plan. Case managers will check with clients and families one week and one month after discharge to see if further services are needed. Group therapy topics include drug/alcohol education, 12 step education, communication skills, relapse prevention, stress management, recovery through art and

music, problem solving, DBT skill development, health issues and many more!

Riverside Community Care

Riverside Community Care is an award-winning, non-profit organization offering emergency care, urgent and outpatient services, in-home and residential care, psychiatric day programming, early intervention, employment support and domestic violence services throughout eastern and central Massachusetts.We partner with local hospitals and physicians, and we work with most insurers and MassHealth. And, Riverside is where your community, schools, and workplaces turn when faced with the emotional aftermath of tragic events such as teen suicides or natural disasters.

Spectrum (partnering with Massachusetts Department of Corrections)

In addition to treatment campuses in Westborough and Weymouth that offer counseling services and medication-assisted, Spectrum also contracts with the Massachusetts Department of Correction to provide a range of addiction treatment and recidivism reduction programming to inmates throughout the state prison system. Our highly effective programs teach participants the skills needed to achieve a law-abiding life, free from drugs and crime. Numerous studies have demonstrated the success of Spectrum’s approach in reducing relapse and recidivism.

South Bay Community Services

South Bay Community Services provides outpatient services to individuals ages 3 and up in a variety of settings. They can see clients primarily in their home, but also office, some schools, and other settings that offer a private space to maintain their confidentiality. They provide individual therapy, couples/family therapy, and medication management. The Worcester office services the majority of Worcester County but there are offices located throughout the state.

Sunspire Health Spring Hill is a residential treatment facility located in Ashby, Massachusetts for men and women seeking inpatient treatment for substance use and co-occurring mental health disorders. With 32 beds available to patients, our facility offers rustic, charming accommodations at our secluded campus set on 70 acres of pristine woodland.

Y.O.U, Inc.

Y.O.U., Inc.’s Adolescent Substance Abuse Services include a continuum of treatment options including assessment, intensive outpatient groups (IOP/SOAP), outpatient groups, relapse prevention,individual and family therapy, and drug screening. In addition to the foundation bearing the initials of Alyssa Elizabeth Dunn, it also stands for Assist, Educate and Defeat.The A.E.D. Foundation, Inc. is committed to Assisting individuals on their road to their recovery as well as their families. Educating the community by bringing awareness of the disease.Defeating the stigma that is associated with drug addiction. ​The A.E.D. Foundation provides all of our services free of charge. This includes all recovery assistance, community education initiatives and the operation of Alyssa's Place: Peer Recovery and Resource Center.

Central Mass Recovery Learning Community (Kiva Center)

The Kiva Center is the connection center of the Central Ma Recovery Learning Community. Their main focus is on promoting wellness and resilience through mutual support. The Kiva Center believes in every person’s right to mental health and ability to achieve it. They believe that relationships are at the center of what heals people who have experienced mental/emotional health challenges. Central Ma Recovery Learning Center is represented by Stephanie Mero. She is a Certified Peer Specialist and Recovery Coach and actively worked on Governor Baker’s State WithoutStigMA campaign.

“Chart” Program – Health Alliance Hospital Leominster

Works with patients who after the ED that have a primary or secondary diagnosis of mental health or substance abuse. Meghan is a Family Support Counselor and Consultant for several school districts within Central and North Central MA. Her program has facilitated discussion with districts regarding the upcoming implementation of the SBIRT in their schools. Meghan also provides individual, family, and group therapy at a group practice in Worcester, Enlightened Interventions.

Heroin Anonymous

Heroin Anonymous (H.A.) is a non-profit fellowship of men and women who have found a solution to heroin addiction, where participants abstain from all drugs and alcohol.Many H.A. members are recovered heroin addicts who meet regularly to help each other stay sober, where they say the only requirement to join is the desire to stop suffering from addiction.Contact: Jackie Rosario

Mission: Our team of youth training and systems specialists provide technical assistance and training to youth-serving systems and state agencies across Massachusetts. Our Mission is to promote access to a continuum of treatment options for youth and young adults with substance use disorders. We work in partnership with the Bureau of Substance Abuse Services and other stakeholders to help youth-serving agencies increase their ability to identify and address substance use issues. We also aim to promote inter-agency collaboration to identify barriers and develop more effective prevention and treatment services for youth and young adults.

Pathways for Change/Sex Trafficking Outreach

Pathways For Change addresses the impact of sexual violence by providing quality and multicultural services to those whose lives have been impacted by sexual violence and to provide education geared toward ending violence. Pathways for Change is funded by the WDPH. Nicole serves as the Sex Trafficking Outreach and Survivor support person at Pathways and works with women involved in prostitution, women who have exited, and with younger women on prevention of Commercial Sexual Exploitation. Nicole is a referral source for these survivors and also provides court advocacy and 1:1 rape crisis counseling. As the Founder and Director of Living in Freedom Together (LIFT), (a survivor led agency) she provides resources, advocacy, and support to victims and survivors of Commercial Sexual Exploitation. The LIFT group in Hamden County House of Correction for incarcerated survivors with SUD connects services prior to discharge from prison A drop in center provides women with a hot meal, clothing, condoms, hygiene products, and has HIV and STI testing on site and provides Narcan to all clients. Community partnerships allow for assistance with accessing treatment if requested.

Learn to Cope is a non-profit support network that offers education, resources, peer support and hope for parents and family members coping with a loved one addicted to opiates or other drugs. Founded by Joanne Peterson in 2004, the organization has grown to include over 10,000 members, and has become a nationally recognized model for peer support and prevention programming.

Worcester Fair Housing Project (WFHP)

The WFHP is a grant project between the City of Worcester Office of Human Rights and Community Legal Aid. We work to protect residents of Worcester County = Western MA from discrimination in housing. As it relates to the DA’s Opioid Task Force, it is important to note that individuals in recovery ARE members of a protected class, and therefore cannot be denied housing, evicted, or in any way treated differently because they are in recovery. Our Father’s House, Inc.

Our Fathers House, Inc. provides shelter and related services to homeless men, women and children.The Emergency Shelter, 55 Lunenburg St., Fitchburg, provides meals, shelter, housing search, and advocacy services in the spirit of hospitality to sober homeless men and women. The Elizabeth House, 76 Mechanic St., Fitchburg is a transitional program for homeless women in recovery. The West Street Sober House, 42West St., Leominster, providespermanenthousing to men in recovery and is modeled after the National Oxford Sober House.Transitions at Devensis a family housing program with 13 units at Devens and 7 in Fitchburg. This program is for single, female-headed households who are homeless with children. The Street Outreach Program offers referral information, crisis intervention, counseling and case management to the unsheltered homeless.

Central Massachusetts Tobacco-Free Community Partnership

The Central Massachusetts Tobacco-Free Community Partnership, a program of the UMass Department of Psychiatry, enhances state and local tobacco control efforts by exposing tobacco industry tactics, mobilizing the community to support and adopt evidence-based policies, and changing social norms that prevent youth from starting to smoke and help current smokers to quit. These comprehensive efforts reduce death and disability from tobacco use. Funded by the Massachusetts Department of Public Health Tobacco Cessation and Prevention Program, the Community Partnership serves as a resource for local coalitions, health and human service agencies, municipalities, and workplaces on tobacco intervention efforts.

Fitchburg Board of Health (Montachusett Public Health Network)

The director of the Fitchburg BOH, Stephen Curry, is Chairman and Lead Agency of the Montachusett Public Health Network (MPHN) consisting of 11 towns (Fitchburg, Leominster, Gardner, Phillipston, Princeton, Westminster, Royalston, Athol, Sterling, Clinton, and Templeton). The MPHN provides Inspectional, Nursing (Communicable Disease tracking), and many other services such as Medication and Sharps Drop off Kiosks in all eleven communities. The MPHN also has a MOAPC (Mass Opioid Prevention Collaborative) grant subcontracted to LUK to provide Opioid Prevention Services to Athol, Fitchburg, Leominster, Gardner and Clinton. The MPHN also has two SAPC (Substance Abuse Prevention Collaborative)grants and 5 towns belong to SAPC-East and 6 towns belong to SAPC-West within the MPHN and these grants are also subcontracted to LUK, Inc.
