Transitional Housing Assistance Funding
Semi-Annual Program Report
The WCADVSA is required to semi-annually report to the Office on Violence Against Women on the following questions. These questions are in addition to the questions in the ‘Monthly Progress Report and Reimbursement Request Form.” All programs need to complete and submit this form to the WCADVSA every six months (see reporting schedule below) even if you have not utilized the Transitional Housing funds allocated for your program.

All Transitional Housing grantees are required to report on the number of victims requesting transitional housing funds whom we were unable to serve, as well as the reason they were not served. Please consider the following definitions as you gather and report this data.
Served – People who received the service(s) they requested, if those services were provided under your Transitional Housing Program grant.
Partially Served -- People who received some services(s), but not all of the services they requested, if those services were provided under your Transitional Housing Program grant.
Not Served – People who sought services and did not receive the service(s) they were seeking, if those services were provided under your Transitional Housing Program grant.
