In my professional opinion, to the best of my information, knowledge and belief, the Special Inspections required for this project, as itemized in the Statement of Special Inspections submitted for permit, have been performed and all discovered discrepancies have been reported and resolved.

Interim reports, documents tagged as containing Non-Conforming work and all Final Reports created for this project and uploaded to the meck-si.com website, are to be considered an integral part of this letter.

I understand that the Department will not allow a Final Inspection request until this document is uploaded to meck-si.com and the *Meck-SI Administrator has been notified to review this document online.
