6) BRIEF RESUME OF THE INTENDED STUDY:

6.1) NEED FOR THE STUDY:

The present day world is full of stress and strain with increasing competitions in all walks of life. This has led to many diseases which though do not kill a person, but hamper ones day to day life. Apabahuka is one such disorder where in locomotive functions are affected.

Apabahuka2 is considered as a disease that affects the Amsa Sandhi and is produced by the Vata Dosha. Acharya Susurutha and others have considered Apabahuka as a Vatavyadhi.

Ayurveda excels greatly in Vata Vyadhi Chikitsa. Therapies for Vata Vikaras like Snehana, Svedana, Basti and Virechana holds good in the case of Apabahuka also2,8. Moreover, Svedana is especially mentioned in the treatment of Apabahuka. Regarding a medicament, it should be less expensive, easy to administer, cause minimal discomfort to the patient and aid in early recovery. Therefore, Eranda which is widely available and renowned for their Vatahara properties is considered6. Incorporating this drug in the form of Nadi Sveda10 and Pinda Sveda11 are considered for the present work. Hence a study is planned with Eranda Pathra Nadi Sveda and Eranda Patra Pinda Sveda in Apabahuka cases.

6.2) REVIEW OF LITERATURE:

· Nidana similar to that of Samanya Vata Vyadhi1.

· Samprapthi,Lakshana’s and Chikitsa of Apabahuka are mentioned by different Acharyas2.

· Treatment measures of Samanya Vata Vyadhi are the measures for Apabahuka also3,4.

· Nadi Sveda and Sankara Sveda(Pinda Sveda) are line of treatment in Samanya Vata Vyadhi5.

· Eranda Patra is having Vatahara effect6.

· Review of such conditions mentioned in contemporary medical science will be undertaken. Relevant journals, periodicals, magazines, books and research conducted at different research centres will also be reviewed.

6.3 OBJECTIVES OF THE STUDY:

a) To study about Nadi Sveda and Pinda Sveda in detail.

b) To evaluate the effect of Nadi Sveda and Pinda Sveda with Eranda Patra in the management of Apabahuka.

c) To compare the effect of Nadi Sveda and Pinda Sveda with Eranda Patra in the management of Apabahuka.

7. MATERIALS AND METHODS:

7.1 SOURCE OF DATA:

(a) Subjects:

Patients with classical features of Apabahuka will be selected from O.P.D and I.P.D of Muniyal institute of ayurveda medical science, Manipal.

(b) Literary:

Literary aspect of study will be collected from classical Ayurvedic and Modern texts books & will be updated with recent medical journals and internet.

The trial drugs will be collected from the source after proper identification.

(d)Preparation of the medicine:

Ingredients of the Medicine:

Group (a) – Nadi Sveda9- Eranda Patra, Water.

Freshly collected leaves added with sufficient quantity of water will be heated and used for the treatment. Treatment will be performed with the help of Nadi Yantra.

Group (b) – Pinda Sveda12 – Eranda Pathra, Tila Taila.

Erenda is cut in to small pieces and is fried with 200 ml of tila taila in a round

bottom vesse to make it plaint. This fried leaves are put in a cotton cloth, free

ends are folded and tied with a thread, thus pinda(pottali) is prepared.This

pinda is heated in oil and is used for sveda.

7.2 METHOD OF COLLECTION OF DATA :

(a) Study design:

A Comparitive clinical study.

(b) Inclusion criteria:

1. Patients suffering from classical signs and symptoms of Apabahuka2 will be selected.

2. Patient fit for Svedana Karma.

3. No discrimination of gender, chronicity or socio economic status.

4. Patients in between 20yrs and 70yrs of age will be selected.

1. Patients with other serious systemic disorders.

2. Age group below 20yrs and beyond 70yrs.

3. Conditions like Spondylolisthesis, Spinabifida, Tuberculosis of spine, Ankylosing Spondylitis, Spinal canal stenosis, Vertebral fracture and Malignancies.

(d) Diagnostic criteria:

· Diagnosis will be purely based on Ayurvedic symptomatology (Nidanapanchaka)1,2. Signs and symptoms of Apabahuka are Amsa shosha, Bahu Prasandidahara, shoola.

(e) Treatment plan:

Patients will be assigned into two groups consisting of 20 Patients in each group suffering from Apabahuka. Both the groups will be given Abhyanga7 as a Pre Operative Procedure with Murchitha Thila Taila for 5 min.

· Group A: The patients of this group will be subjected to Nadi Sveda with Eranda Pathra for 30 min daily.

· Group B: The patients of this group will be subjected to Pinda Sveda with Eranda Pathra for 30 min daily.

Both the groups will be treated for a period of 7 days.

(f) Posology:

External application.

(g) Follow up and Duration (Both A & B Group):

The patients will be followed up at regular intervals of 15 days after treatment for a period of 30 days.

(h) Assessment criteria:

The subjective and objective parameters of pre and post treatment will be compared for assessment of the results. Then all the results will be analysed statistically.

Subjective parameters:

- Pain on rest

- Pain on movement

- Stiffness

- Radiating pain to shoulder and arms

Objective parameters:

- Tenderness

- Cervical flexion

- Cervical extension

- Right lateral movement

- Left lateral movement

- Rotation

7.3. DOES THE STUDY REQUIRE ANY INVESTIGATION OR INTERVENTION TO BE CONDUCTED ON PATIENT OR OTHER HUMANS OR ANIMALS? IF SO PLEASE DESCRIBE BRIEFLY.

The study will be conducted clinically. No animal experimentation will be carried out.

Investigations: For diagnostic purpose (if necessary)

· Blood Investigation : Hemoglobin gm%, Total WBC Count, Differential WBC Count and Erythrocyte Sedimentation rate.

· Biochemical investigation : Fasting Blood Sugar

· Urine : Sugar, Albumin, Microscopic

· X - Ray of Cervical Spine - AP and Lateral views(If necessary)

7.4 HAS THE ETHICAL CLEARANCE BEEN OBTAINED FROM YOUR INSTITUTION?

Yes, the clearance has been obtained through the ethical committee formulated

in the institution.
