Your health is important, and we would like to help you reach your wellness goals. Our records indicate that you are at risk for Pre-Diabetes, which automatically grants you enrollment in DriveTime’s Diabetes Wellness Program. In partnership with United Healthcare, we are pleased to offer this simple program to help you save money and improve your health.
Enclosed please find a Member Guide to help explain the program and outline the resources available to help you manage your condition. By meeting your compliance requirements by August 31, 2013, you will be eligible to remain in the “Diabetes Wellness Program” in 2014. Your enrollment in this valuable program will allow you to reduce your out-of-pocket expenses related to Pre-Diabetes.
Why am I receiving this?
You are automatically enrolled in the “Diabetes Wellness Program” if you have been identified as being at risk for Pre-Diabetes; this is based on United Healthcare’s proprietary process in which they review 40 Diabetes or Pre-Diabetes related risk measures.
This is news to me. Is there any way I could learn more about why I’m included?
United Healthcare is reviewing 40 risk measures in making this determination. They actually have access to more data and health information than your physician. If you would like to receive more information about why you were selected you can reach out to UnitedHealthcare Member Services at: 1-866-844-4864.
Why is it important?
Since you have been identified as being at risk, you still have the opportunity to take preventative measures against Diabetes. The Diabetes Wellness Program shows you how you can manage your health and receive additional benefits. Traditional Plan members will have reduced plan costs for Diabetes-related doctor visits and select medications and supplies. Liberty Plan members will receive HSA contributions to reduce these Diabetes-related out-of-pocket expenses. We strongly encourage you to participate in this plan, and take advantage of all the resources it makes available.
Can I opt out of the program?
The choice is yours. If you do not wish to participate in the program you can complete the included Opt-Out form and return it to United Healthcare by March 15, 2013. Forms should be mailed to:

What do I need to do?
Be sure to register on the HealthInsight Member Portal
HealthInsight™ – uhc.healthinsight.com/drivetime– is the command center for the Diabetes Wellness Program.HealthInsight makes it easy to track your progress by automatically recording your doctor visits and lab tests. You should check this site throughout the year to determine the health actions you complete to improve or monitor your health and be eligible for the program’s benefits.
To access your scorecard, simply register at uhc.healthinsight.com/drivetime. First time users will need to enter the following information:
- Member ID (located on your UnitedHealthcare Plan ID) card)
- First Name
- Last Name
- Date of Birth
- Zip Code
What if I have already completed one or more of these actions/compliance requirements?
That’s terrific! You will receive credit for any actions/compliance requirements you completed back to September 1, 2012.
How do I read my scorecard?
Your scorecard lists the health actions you complete to remain eligible for the Diabetes Wellness Program benefits. It tells you when each action must be completed, and tracks your progress.
A greenin the right column shows the health actions you have completed.
A red ! in the right column shows the health actions you still need to complete.
