On page 2, line 10 by deleting the rest of the line after "attendance"; and
On page 2, by deleting line 11; and
On page 2, by deleting everything on line 12 except for the period; and
On page 2, line 24 by deleting everything after "requirements"; and
On page 2 by deleting line 25; and
On page 2 by deleting "papillomavirus,"; and
On page 3 line 2 after "available" by inserting "information"; and
On page 3 line 2-3 by deleting "any forms that reflect a parent's choice to accept or decline" and inserting in lieu thereof "regarding"; and
On page 3 line 5 after "year" by inserting "so that the parent or guardian may decide whether he or she wishes to have his or her child immunized against human papillomavirus. All public or private elementary or secondary schools shall mail information concerning immunization against the human papillomavirus to each parent or legal guardian upon his or her child's enrollment in the sixth grade"; and
On page 3 after line 5 by inserting the following:
"(7) Notwithstanding any provision of the Kentucky Revised Statutes to the contrary, a parent or legal guardian of a child shall never be required to opt-out of a required vaccination for the human papillomavirus. A parent or legal guardian of a child shall always be privileged to decide whether he or she wants his or her child to be immunized against human papillomavirus, and shall only be requested to opt-in to a vaccination program for the human papillomavirus for his or her child."; and
On page 3 by deleting lines 6 through 13 in their entirety; and
On page 4 by deleting lines 1 through 10 in their entirety.
