DATE:August 15, 2007
SUBJECT:Creation ofTheKentuckyChild Support Program(PAM-CS-190) and Procedural Instructions
PAM-CS-190 and its procedural instructions have been created to update information and to condense the following three pamphletsinto one comprehensive pamphlet: What Is Child Support? (PAM-CS-107), Moms, Dads, and Paternity Establishment -Kids Need All Three (PAM-CS-48) and Paternity Testing - Answers to Frequently Asked Questions (PAM-CS-18). PAM-CS-190 and its procedural instructions are on the CHFS Intranet Forms page. The Spanish version of PAM-CS-190 will be added to the Intranet when it has been translated by the Cabinet for Health and Family Services Language Access Section.
There will be an initial shipment of pamphlets to child support offices. The number of pamphlets shipped has been determined according to caseload. Regional offices will provide pamphlets to IV-A offices and others as requested. After the initial shipment, pamphlets may be ordered by sending a request, in writing, by letter or fax to:
Division of Child Support
P.O. Box 2150
Frankfort, KY40602-2150
FAX: (502) 564-0255
The request must include:
- name and number of the pamphlet
- name and full address of the requesting child support office
- exact number of pamphlets needed
