Case Manager
ACCELERATED LEARNING & LIVING (A.L.L.)

The Case Manager of the Accelerated Learning and Living program works under the supervision of the Associate Director and includes overseeing the transitional housing component. The Case Manager is responsible for managing and maintaining close contacts with the HIS Facility Manager,Property Management, resident manager and resident families; and cooperating with other staff in maintaining the mission of Harbor Interfaith Services. Duties include but are not limited to the following:
Responsible for the program intake process to ensure the most efficient model for screening applicants and minimizing the turnover rate.
Develop and implements the goal-setting process, to assist residents with setting realistic personal, educational, and employment goals and monitor their progress of goal attainment.
Provides case management services, bi-weekly to assist residents in removing barriers to achieving goals. Provides crisis intervention if indicated.
Thoroughly document services provided to residents in case files. Documentation to include quarterly goal statements and progress toward goal achievement.
Maintains the A.L.L. client tracking system and provides systematic reports to the Executive Director.
Work with the Executive Director in development of project objectives to support Harbor Interfaith Service’s organizational goals, especially with regard to the Accelerated Learning & Living program.
Ensures the development and implementation of classes for residents.
Assists with presentations to increase knowledge and awareness of the Accelerated Learning & living program.
Represents Harbor Interfaith in educating the community about the Accelerated Learning & Living program.
Coordinates with the Director of the Family Resource Center and the SPA 8 Homeless Families Solutions System.
Coordinates with the Director of Educational Services to recruit community volunteers to serve as facilitators for workshops/ seminars.
Ensures that apartment building is in good repair and free of graffiti. Conducts weekly inspections and promptly coordinates maintenance and repair issues as needed and within budgetary constraints.
Works a flexible full-time schedule which, due to the nature of this position, may include evening, weekend and/ or early morning hours.
Assumes other responsibilities as assigned by the Executive Director
Requirements:
- Minimum five years experience in working with “at risk” population. “At risk” should be defined as parents and children who have experienced significant trauma, domestic violence, displacement/homelessness, and/or a variety of other deprivations which has had a powerful negative impact on the family unit
- Demonstrated ability to design, implement and evaluate human services programs
- Excellent communications skills, both verbal and written; must submit writing sample. Strong interpersonal, negotiation, conflict resolution and leadership skills
- Working knowledge of San Pedro/Harbor Area educational, employment and counseling resources required; knowledge of local mental health, legal, medical, and financial resources desirable
- Working knowledge of word processing and computer applications
- Management style characterized by initiative, flexibility, creativity, and strong organizational skills desirable
- Advanced degree in social work or human services management desirable
- Minimum of three years experience in Job Development/Placement
