Dear Parent/Caregiver,

Your son/ward has been selected to compete in the Bankstown Zone Swimming Carnival. All students are expected to catch the bus to and from the carnival. The only exception will be when parents drop and pick up their son from the venue. Please indicate clearly on the consent form how your son will be attending the venue.

Listed below is the full detail of the carnival:

Date / Monday 2ndMarch, 2015
Venue / Roselands Aquatic Centre - Centre Ave, Roselands NSW 2196
Times / Depart from the school at 8.00amand return to the school at 3.00pm. Students will be dismissed on their return to school.
Cost / $12(entry fee and bus fare)
$5(entry fee)
Money is to be paid to the front office by Friday 27th February, 2015.
Travel / All students to travel to and from the carnival by bus under the supervision of teachers.
The staff members with first aid training / Mr Carson and Ms Wheatley
Accompanying staff members / Mr Carson, Ms Wheatley and Mrs Barber
Students will need to bring / Bag, hat, sunscreen, water bottle, swimming costume and towel.
Food / A sausage sizzle will take place during the day and some canteen facilities will be available, however, students are encouraged to bring their own lunch.
Clothing / Students must wear full school sports uniform or they will not be permitted to attend.
FULL SPORTS UNIFORM MUST BE WORN.
MEDICAL INSURANCE
Medical Insurance: Parents please note there is no personal injury insurance cover provided by the NSW Department of Education and Training for students in relation to school sporting activities, physical education lessons or any other school activity. Parents and caregivers are advised to access the level and extent of their child’s involvement in the sport program offered by the school, school sport zone, area and state school sport associations when deciding whether additional insurance cover, above that provided by Medicare, is required. Personal accident insurance cover is available through normal retail insurance outlets.
The NSW Supplementary Sporting Injuries Benefit Scheme, funded by the NSW Government, covers any injury resulting in the permanent loss of a prescribed faculty or the use of some prescribed part of the body.
