SPORTS FACILITY EVENT REQUEST FORM
*Campus Recreation does NOT reserve facilities to non-affiliates. This form must be 100% completed prior to submission. We will not accept or even review incomplete forms. Please allow up to two weeks for approval or denial. Undergrad Student Organizations also need content approval from the SLIC and recreation will NOT approve your event unless we have the approval from SLIC.

If yes, continue on the step V.
If no, stop! Do not submit this form unless you have pre-approval from SLIC! Campus Rec cannot accept any reservation requests from undergrad student organizations until the SLIC has forwarded on the event content approval form. This is an automated process if you properly filled out the SLIC form. Please see your group’s advisor if you have any questions about the SLIC process.

*For items with a *, Campus Rec will NOT provide these! It is up to your group to make the appropriate arrangements with Campus Scheduling or Media Services. Campus Rec is NOT an event planning company. We just provide the facility.
Please note that if the area is not cleaned after the event, a non-negotiable cleaning fee (minimum of $100) will be applied to the organization or department.
Campus Rec will not support parking assistant requests. All parking requests should go through parking services.

CANCELATION POLICY:
Campus Recreation reserves the right to charge the listed organization or department a $100 cancelation fee or charge the organization or department for damages, missing items or cleaning fees. This fee is subject to the discretion of the department of Campus Recreation and is non-negotiable.

*This form will not be accepted or even reviewed if the POETS code is not included by the listed organization or department. Your organization’s treasurer or your department’s head has this code.
