We have come a long way since our first services opened in Bridgend, supporting the resettlement of people from the long stay psychiatric wards of Parc and Penyfai hospitals into homes in the community. Salary:£18,685 per annum.
Contract: Permanent
Hours:37 hours per week, including some weekend/ evening work and sleep

Holidays:26 days annual leave plus bank holidays.

Project Outline:
The role of the Peripatetic Case Worker is to work flexibly across a number of projects within the Gwent area providing cover for staff absence in both Case Worker and Support Worker roles.
Cover will typically be across our supported housing, tenancy and floating support and crisis intervention projects, although there may also be opportunities within other projects
Supported Housing
We manage a range of supported housing, from 24-hour staffed projects that provide intensive support to services offering tenants the reassurance of support for a few hours each day. Each service is designed to support people in developing daily living skills, in making healthier lifestyle choices, in accessing education, training and leisure opportunities and in taking the next steps towards independent living.
Tenancy/Floating Support
Providing support to people in their own homes, who may be struggling to manage their housing tenancies and financial obligations because of their mental ill health. We provide support around debt management, budgeting and claiming benefits, as well as daily living skills, accessing education, training, employment and leisure opportunities and improving physical wellbeing
Crisis Intervention
Working with people to ensure that their mental health crisis does not result in a housing crisis, and vice versa. We work with patients and staff on mental health wards to ensure suitable accommodation is in place for when a person is ready to leave hospital. We support people who are homeless or at risk of homelessness find accommodation that will support their mental wellbeing.

Job Role:Peripatetic Case Worker
Accountable to:Project Co-ordinator.
Hours: 37 hours per week predominantly Monday to Friday but with some flexibility to cover occasional evening and weekend shifts.
Location: Allocated base to cover Gwent
SettingFlexibility to provide cover across a variety of Gofal project providing a range of support to people around mental health
Job Purpose
- To work flexibly across Gofal projects providing cover for staff absence covering both Case Worker and Support Worker roles.
- To provide a range of social, emotional and practical support to people with mental health and associated problems.
- To provide support and mentoring to Support Workers in the implementation of support plans.
- To comply with and maintain all necessary administrative and reporting systems.
This description outlines the main areas of the role, however no document can accurately cover all duties and the post holder will therefore be expected to carry out other duties commensurate with the scope of the role and across all areas of Gofal operations.
Role Expectations:
Service User Support Work
- To lead on assessment of needs of individuals referred to the projects, obtaining additional information as necessary.
- To effectively manage and maintain one’s own case load to ensure that an effective service is delivered to service users in crisis.
- To work with the service user and, where relevant, to draw up support plans to meet identified needs and ensure support is delivered in line with the support plan.
- To review progress and relevance of risk assessments and support plans and update accordingly to ensure that risk is managed effectively, monitored on a regular basis.
- To involve service users in the development, running and evaluation of the service.
- To liaise with other agencies with and on behalf of service users as part of planned support, to include participation in joint meetings and case reviews ensuring collaboration and effective support planning.
- To support service users to access information and advice in order that they are able to make informed choices in matters that affect them to promote their independence.
Colleagues Support and Mentoring
- To provide support and advice to colleagues in their delivery of support to individuals, where required.
Administrative Requirements
- To keep accurate notes of all support provided in line with funding requirements.
- To ensure that risk assessments, support plans and reviews are maintained up to date.
- To ensure all relevant information is shared with colleagues and other services as appropriate.
- To ensure all project communication methods are used appropriate and effectively.
- To ensure that IT systems are used effectively and in line with organisational procedures – eg. Use of e-mail and database systems.
