PURPOSE
To monitor and provide feedback to staff and volunteers on hand hygiene.
PROCEDURE
Hand hygiene is monitored once/year or more frequently as required.
It can be usedin different ways to monitor hand hygiene:
1) By the supervisor/ manager to observe for, and provide feedback on hand hygiene practices during clinical supervision and performance reviews. As a supervisory tool, the results are to be discussed with the involved participants, to provide positive feedback and/or to plan improvements.
2) As a reflective practice tool for staff self- reporting. When this tool is used in this way, the participant tracks his orher own practice over a period oftime to identify areas that he/she may need to improve for his/her own practice.
3) As a general work place hand hygiene audit. This is done by observing the hand hygiene of health care workers, clients, or volunteers over a defined period of time. When used in this way the information fromthe audit is to be compiled as anonymous data and reported back to participants. The information is to be used to plan andimplement changes to improve hand hygiene.

Enter the initials of the person who was monitoring. Enter the time in which the observations took place.
Enter the name ofthe participant ifthis is to be a supervisory tool or enter the position or type of worker for a general work place audit.

Opportunity Requiring Hand Hygiene: Enter HW for Hand wash or HA for use of a hand alcohol antiseptic for each of the situations observed that are an indication for hand hygiene during the observation period.
The data from the formis to be compiled and reported back to the individual or group as outlined above.
