Patient Participation Annual Report for 2015-2016

To ensure that patients are involved in decisions about

the range and quality of services provided by High Street

Surgery it was decided to continue with the Patient Participation

Direct Enhanced Service. (DES). The DES aims to promote the proactive engagement ofpatients and to seek their views through the use of an annualpractice survey. The outcome of the engagement and views of patients aredetailed in the following report.

Profile of the PPG

High Street Surgery has a medium established active patient group. In an attempt to get a representative spread of age, gender,health issues etc a number of approaches were used to invitemore patients to join.

  • Posters were put up in the waiting room
  • Invites were attached to repeat prescriptions
  • Forms were placed on reception
  • Practice Nurse asked to invite patients with long term

conditions

  • Midwife asked to approach new moms

Annual Survey

The PPG membership worked in collaboration with the Practice to produce an Annual Patient Survey. After being agreed the Survey was handed out to patients attending the Practice. This questionnaire is designed to be handed out randomly to patients coming in to the surgery. Patients attending for appointments were asked if they werewilling to complete a questionnaire it was entirelyanonymous. The results were collated in-house and shared withthe PPG.

PPG Lead and Secretary

Volunteers/Nominations were requested to take the role of PPG Lead and Secretary. It was agreed that Pat Martin would be Lead and Sue Rook will act as Secretary.

Online Services

EMIS Patient Access had been introduced into the Practice. The PPG had been informed of how patients could register for this service so they will get greater access to their medical records online.

Action Plan

The Practice informed the PPG that a check in screen and a call in screen would be installed in the Surgery later in the year so patients would be able to check themselves in.
