Based on monthly evaluation of student progress for the month listed above, progress in the course listed above is unsatisfactory or the WSLP goals were not obtained. As a result, the following Student Success Plan (Intervention Plan) is being developed as a guide to more appropriately meet the student’s needs in obtainment of course and WSLP goals. An in-person attendance requirement has been added to provide the student with an opportunity for success. Failure to follow this plan will result in the student’s withdrawal from the course listed above and/or the Spokane Virtual Learning program.

Description of Intervention(s) and how student progress is to be improved:

Attendance: Student is required to attend his / her school’s iCAN Lab or Homework Center a minimum of two to three days per week and receive validation of that attendance in the form of a signature (see second page). This attendance is REQUIRED to remain enrolled and should be submitted to virtual teachers each Friday. Students who cannot show proof of this attendance will be dropped from the course.

PROOF OF ATTENDANCE

iCAN Lab/Homework Center attendance MUST be verified WEEKLY.

  1. Please work with your SVL teacher and Student Support Specialist to determine location for in-person work time on SVL course: iCAN Lab, Homework Center or SVL Student Support Lab at public libraries.

  1. Fill out the chart below with date, time in/out, official iCAN Lab or Homework Center supervisor signature, and text or email evidence of chart completion to your teacher by Friday of each week.
  2. This chart must be completed and returned to your SVL teacher each school week this month.
  3. This attendance is REQUIRED to remain enrolled. Students who cannot show proof of this attendance will be dropped from the course.
