A copy of this completed form and supporting documentation must be sent to everyone invited to the meeting.
The transfer meeting should make recommendations regarding the EHC Plan. The LA will make the decision whether or not to transfer the Statement of SEN to an EHC Plan.
Completed forms and any additional supporting reports should be sent by email to the Camden SEN Team at within two weeks of the transfer review meeting to enable the LA to issue an EHC Plan within the 20 week statutory timescale.
Emails and reports should be sent encrypted email (i.e. via the Egress system) or password protected, via unencrypted email. The password should be sent in a separate email.
