  1. Lottery Process:
  1. Complete and sign an “intent to register” form.
  2. Submit this “Intent to Register” form to the CAD office (SCI 394) or Nursing Office (BH 357) by 5:00 PM on Friday, September 6, 2013. Forms are date and time stamped to ensure that everyone in the lottery meets the posted deadline.
  3. Emailed forms will not be accepted.
  4. Late forms will not be accepted.
  5. Students in the CAD or Nursing Department on the Cañada campus may deliver their forms as follows:
  6. CAD to Rene Dahl by placing them in her mail box in Bldg 5 Room 228 or by handing them to her during advising hours on Thurs (2-4 pm) prior to the deadline (September 6, 2013)
  7. Nursing to the School of Nursing office in Bldg 18 Room 205 by September 6, 2013.
  8. Sixteen names will be drawn by lottery from all those who have submitted the form on time - eight students from CAD and eight students from nursing.
  9. Students will be notified of their selection by Friday, September 13, 2013.
  10. Students chosen by lottery will have two weeks from the notification date to submit a deposit of $800 in the form a money order or cashier’s check payable to the College of Extended Learning. Payment must be received in the CAD office (SCI 394) or Nursing Office (BH 357) by 5:00 PM Friday, September 27, 2013.
  11. Students in the CAD or Nursing Department on the Cañada campus may deliver their payment as follows:
  12. CAD to Rene Dahl by placing them in her mail box in Bldg 5 Room 228 or by handing them to her during advising hours on Thurs (2-4 pm) prior to the deadline September 27, 2013)
  13. Nursing to the School of Nursing office in Bldg 18 Room 205 by September 27, 2013.
  14. If the deposit is not received by the stated deadline, the student’s lottery position will be forfeited and the next student on the waiting list will be notified and given 10 days to submit a deposit. No exceptions.

DATES: Travel dates –January 6, 2014 to January 20, 2014. Seminar dates – TBA.

TRAVEL INFORMATION:Tours of London, Oxford, Wales, Cardiff and Southhampton. Visits to hospitals, clinics, schools and historical sites.

REGISTRATION MAXIMUM: 20 students (10 CAD; 10 Nursing)

ESTIMATED COST: Approximately $4500 - $5000 including airfare SFO to London; lodging (double or triple occupancy); ground transportation; guides; gratuities; breakfasts; tours of Windsor Castle, Tower of London, Westminster Abby, and other historical sites; and registration fees through the College of Extended Learning which includes required travel insurance.
