Location: Hillside Bridge Health Centre, Bradford.
JOB SUMMARY
An exciting opportunity has arisen for a Senior Advanced Nurse Practitioner to join our innovative team at Local Care Directs primary Care site at Hillside Bridge Bradford.We are looking for ambitious, flexible, independent individuals who will be able to provide a high level of clinicalcare for our GMS patients managing a range of acute, non-acute and chronic disease conditions. The successful applicant will be taking on a senior role as lead ANP for a designated primary care site which will involve working closely with our GP’s, nursing staff and non-clinical staff. He/She will possess an Advanced Nurse Practitioner qualification and be proficientin non-medical prescribing V300 equivalent, other duties will include home visits, dealing with pathology results, correspondence and proving leadership for the nursing team at Hillside Bridge.

Duties and Responsibilities:
Clinical
- Practice, without direct supervision, as an independent primary care Advanced Nurse Practitioner within various clinical environments, participating in care of all age ranges and presenting conditions chronic, acute and non-acute.
- Independently prioritise, assess, diagnose, treat, and safely discharge/refer patients attending with undifferentiated and undiagnosed primary/urgent health care problems. Formulate management plans ensuring relevant follow up.
- To manage daily surgeries comprising of 10 minute face to face consultations, 5 minute telephone consultations and telephone triage work. Undertake home visits and acute visits for certain medical emergencies arranging hospital admission if required.
- Prescribe and review medication as necessary within NMC and local formulary guidance for areas such as chronic disease and long term health needs. This will include prescribing for areas such as asthma, diabetes, Hypertension, COPD, CHD and HRT. This list is not exhaustive and may expand with patient need and clinical experience.
- Act as a referral agent and care co-ordinator by establishing multi-agency collaboration across the primary/secondary interface to meet the needs of the patient.
- Take an active role in the processing of Pathology Links and Post actioning changes to patient care as directed by Secondary Care / other MDT’s.
- To actively lead, formulate plans and contribute to the achievement of revised Quality outcomes Framework (QoF) indicators.
- Actively participate with the implementation of Clinical Enhanced Services as directed by the CCG and Practice.
- To provide clinical leadership and support to the Practice Nurse and team of Health Care Assistance performing annual PDR’s
- Work within the parameters of current nurse prescribing legislation and NMC code of Conduct.
- Work with clinical and non-clinical team to ensure that the LCD makes a significant contribution to health improvement and the reduction of health inequalities. Use His / Her individual skills and experience in order to make a positive contribution to the organisation and the development of the ‘one team’ approach
- Ensure the nursing profession contribute to the LCD vision for integrated, high quality and patient focused health services.

Research and Quality

Management and Leadership
- Lead team meeting’s and contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the team’s performance.
- Actively promote integrated working in collaboration with other health care professionals both within and outside the LCD organisation.
- Promote effective communication and relationships within the team
- In collaboration with Senior Nurse Manager and Lead GP’s actively contribute to the development, implementation, review and evaluation of clinical protocols. Take lead role in encouraging teamwork and sharing best practice.
- Ensure effective communication, participating in meetings and liaising with other professionals, services and agencies as required.
- Help develop clinical and business strategy working closely with senior primary care management.
- Work to improve and implement clinical and business development KPI’s (Key Performance Indicators).
- To act as the site clinical governance lead and to facilitate implementation of key clinical governance processes within the surgery as directed by the central clinical governance team.
Professional and Educational
- Develop own skills and competencies through education, training and development activities related to the current and future demands of the role.
- Understand and observe professional and national guidelines on confidentiality.
- Comply with the corporate governance structure in keeping with the principles and standards set by the organisation.
- Ensure clinical supervision needs are met, and that Nursing staff are supported with revalidation.

Infection Prevention and Control
- Healthcare workers have a duty of care to patients and are expected to comply fully with the companies Infection Prevention and Control (IP&C) policies and attend/complete yearly IP&C training, for personal and patient safety and for prevention of healthcare-associated infection (HCAI). Knowledge, skills and behaviour in the workplace should reflect this.

- Evidence of continuing care for a cohort of patients ie specialist clinic such as Asthma or CVD.
