RE: Violation of Procurement/Travel Card Policies and Procedures

This is to inform (individual(s)) thatrules or procedures as outlined in the Purchasing/Travel Card Program Cardholder’s manualand/or Travel Policy per FMC 2017-2 have been violated. Specifically, the (policy/rule) relating to (policy/rule reference) was violated by (individual(s)) on (date) when (description of violation).

This violation is the (# Offense) offense for (individual(s))for accountending in –(last four).

[FIRST OFFENSE] This written warning is notifying(individual(s)) of the need to alter conduct and to follow all Procurement Card/Travel Card Program policies and procedures. Please submit a signed copy of this notice to by (date)acknowledging your understanding and willingness to follow policies and procedures on future transactions.

[SECOND OFFENSE] This violation will require (individual(s)) to re-take and pass the Procurement Card online trainings (1. Program Overviewand 2. Verifier Role Stage 1) and submit current proof of completion to with a signed copy of this notice by (date).

[THIRD OFFENSE] This violation will result in needing prior Agency Program Administrator (APA) approval documentation before completing any purchases for three (3)months.If the APA is in violation, the APA must seek IDOA approval before completing any transactions. The approval process for this arrangement must be presented to and approved by by (date).

[FOURTH OFFENSE] This violation will result in permanent suspension of your Procurement Card/Travel Card. (Individual) is required to submit the card to (APA or IDOA) effective immediately; a signed copy of this notice acknowledging card termination must be submitted to by (date).

Additional incidents of failure to follow all Purchasing Card/Travel Card Program policies and procedures may lead to further action, up to and including the permanent suspension of card or program usage. IDOA maintains the right to use discretion when determining the most appropriate course of action following an offense. IDOA also maintains the right to pursue disciplinary action against the most appropriate individual or group as it relates to the infraction.
