Your child, name of student, has been involved in the following incident(s):

·  Your child is suspended from school for Number school days effective Month Day, Year, and running through Month Day, Year.

·  Add any other disciplinary action.

Should school be closed due to inclement weather or other cause, the suspension dates will be accordingly adjusted.

You and your child may request the assistant superintendent, Dr. Cherie Whitehurst, to review upon the written record this action. Such request must be made in writing to Dr. Whitehurst within three (3) days of your receipt of this notice. Failure to make a timely request will constitute a waiver of the right to such a review. During the appeal process, the suspension will remain in place. The review of the assistant superintendent is final. The written notice of appeal may be sent to the following address:

I regret to advise you that I must also recommend that name of student be expelled for the reasons stated above and that your child be suspended pending action by the School Board on my recommendation of expulsion. You and your child have the right to a hearing before the assistant superintendent regarding this recommendation. Should you or your child desire such a hearing, you must so advise the assistant superintendent, in writing, within five (5) days after receipt of this notice. If no timely request for a hearing is made, the assistant superintendent will act on my recommendation without further notice to you and without a hearing. If a timely request is made, you and your child will be notified of the date, time, and place of the hearing and the procedures to be followed at the hearing.

No community-based educational alternative education or intervention programs are provided during the suspension, however, your child may make up any class work missed during the suspension from school.

Your child is prohibited from coming on school property or attending any school sponsored activities while on suspension. Violation of this prohibition shall be considered trespass and may result in prosecution. Your child is, however, permitted to attend and participate in any and all hearings concerning the offense and/or disciplinary consequences.
