YKC- Intensive In-Home, service children and adolescents from age 5 to 17 years with mental health and /or behavior issues, with severely impair functioning at home, school and in the community.

YKC - Mental Health Skills Building, service adults from the age of 18 and older with mental, behavioral and emotional difficulty who present a need for greater stability and independence.

To assist youth and their families utilizing a nonresidential community-based service to build and maintain a nurturing, supportive, healthy and caring living environment by using knowledge, skills, professional experiences, positive communication and educational techniques to reduce the risk of out-of-home placement and allow youth to function positively and add value to their home, school and community.

INTENSIVE IN-HOME COUNSELING:

YES KIDS COUNT, Clinical team, with the client, creates individualized service plans designed to meet the specific needs of each of our client. Our QMHPs provide the following services under the tutelage of our clinical staff and receives direct weekly supervision to ensure a high level of clinical interventions:

YKC, Provide therapeutic counseling services for the individual and the family that stabilize and improve client and family functioning. Prevent out of home placement and transitions youth that are returning to the home, school and community.YKC, Provides case management and crisis interventions as well.

YES KIDS COUNT (YKC), dedicated team members and Qualified Mental Health Professional (QMHP) are deeply committed to providing supportive and intensive community base intervention to youth who exhibit, at least, two of the following patters of behaviors:

1. Difficulty establishing or maintaining normal interpersonal relationships to such a degree that they are at risk of hospitalization or out-of-home placement because of maladaptive behavior and conflicts with family members and peers in the community.

2. Inappropriate behavior that repeated interventions by agencies such as mental health, social services or the judicial system are needed.

3. Difficulty with age appropriate cognitive ability to a degree that they are unable to recognize personal danger or inappropriate behavior that impede their ability to engage in pro-social interactions with peers, parent/guardian and authority figures.

4. Difficulty expressing a wide range of emotions to such a degree that interactions with parents and authority figures results in oppositional, aggressive and threatening behaviors.

5. Individuals with a history of psychiatric hospitalization and on medication within the past twelve months.

Our Clinical team, along with the client, creates individualized service plans designed to meet their (the client) specific need. Our specialists provide structured case management such as;

Our Experience Clinical team,Provide structured support and case management assistance to adults with mental conditions who demonstrate a need for greater stability and personal management.Our specialist assist in coordination of care for our clients by encouraging their compliance with community base services to include:Psychiatrist; Primary Care Physician; Out Patient Therapy; and Probation and Parole.
