Hospice Palliative Care Regional Program Council
Background and Mandate
The Ontario Palliative Care Network (OPCN) is an organized partnership of community stakeholders, health service providers and health systems planners. Together with Regional Palliative Care Programs, the OPCN will develop a coordinated, standardized approach to high-quality, sustainable and person-centred hospice palliative care for all Ontarians, regardless of age or disease type. The role of the OPCN and the Waterloo Wellington Regional Palliative Care Program is to ensure implementation of ‘The Declaration of Partnership[1], priorities at both the provincial and regional levels. Specifically, the WW IHPC Regional Program Council:
- Is the principal regional advisor on high quality hospice palliative care and will provide advice to the WW LHIN and the WW Regional Cancer Program on planning, oversight and leadership for regional HPC program activities.
- Provides oversight and adviceto the Waterloo Wellington Hospice Palliative Care Regional Program for quality improvement, data and performance measurement and acts as a central coordinating body to ensure that the resources and activities within the WW IHPC system are coordinated, effective, integrated, ethically sound and are contributing to the system’s goals in a timely manner.
- Supports regional implementation of high-quality, high-value hospice palliative care by advancing standardization, education, coordination and continuous quality improvement across all sectors within the region based on best practices in accordance with the OPCN direction and the Declaration, and aligns with both the LHIN Integrated Health Services Plan and the Regional Cancer Programs Strategic Plan.
The Waterloo Wellington Integrated Hospice Palliative Care Regional Program & Council
The Waterloo Wellington Integrated Hospice Palliative Care (WW IHPC) Regional Program is responsible for supporting excellent hospice palliative care services and for ensuring that that these services will be accessible to the people of Waterloo Wellington Local Health Integration Network (WWLHIN), regardless of place of residence, diagnosis or care setting, consistent with the Canadian Hospice Palliative Care Associations (CHPCA) Model to Guide Hospice Palliative Care (2013).
The goal of the Waterloo Wellington Integrated Hospice Palliative Care Program Council is provide advice to inform the planning, implementation and evaluation of the WW IHPC Regional Program to optimize the delivery of hospice palliative care services in Waterloo Wellington. The Council will identify strategies that will improve the LHIN-wide hospice palliative care system through application of best evidence to improve patient experience and equitable access to a high quality standard of care.
In order to “transform palliative and end of life care[2]” the membership of the WW IHPC Regional Program Council is evolving to include a comprehensive collection of critical skills that will support innovation, thought leadership and an objective, regional patient-centered perspective. Utilizing generative questioning and informed by patient and system outcome performance data, the skills-based WW IHPC Regional Program Council will embolden transformation of the WW IHPC Regional Program by providing advice regarding program strategy formulation and execution and resource allocation in support of provincial (OPCN) and regional (WWLHIN IHSP) health system priorities.

The WW IHPC Regional Program is seeking Expressions of Interest (EOI) from regional stakeholders, who possess one or more of the following skills, to join the WW IHPC Regional Program Council:

The WW IHPC Regional Program is seeking volunteer members who have the skills and ability to influence change and reflect issues/concerns from regional stakeholders, including families and caregivers and who possess an understanding of health care at the regional and provincial level. Members should not represent a specific constituency but are invited to bring their expertise and a true desire to advance the system to the table (not just one sector, service, professional interest or geographic area).
Selection of council membership will be competency based, representing a balance of skills and expertise needed to support the strategic objectives of the WW IHPC Regional Program. Additional considerations will include demonstrated experience contributing to provincial and regional strategic priorities and balancing of sectorial and sub-regional representation.
Members of the WWIHPC Regional Program Council are volunteers. The Council will meet 6-10 times a year. Meeting times and locations will be adjusted to support attendance. Attendance at meetings will be facilitated with technology.
