Appendix 1 – Formatting Requirements

RFP Checklist for Vendor Proposal Contents and Responsiveness (Tabbing Structure)

Bid proposals that do not adhere to the following format may be deemed non-responsive. It is strongly encouraged that these additional formatting requirements be reviewed prior to submitting bid proposals. Vendors must respond to all sections of the RFP. Failure to respond to each section in each Article may result in disqualification from the bidding process. Vendors shall follow the tabbing structure below when preparing their binder’s. Bidders must verify that their Proposal is submitted to the appropriate location on time per the schedule of the RFP.

Tab Name Bidder Requirements/Checklist for Technical Proposals

Technical Solution Binder / DMB 285 / The Bidder shall provide DMB 285 (Letter of Submittal) with the following statement in the pricing field “See Attachment A”. This must be signed by an individual authorized to legally bind the Bidder.
Executive Summary / The Bidder shall provide a brief overview (not to exceed 4 pages) of the entire bid package proposal.
Attachment 1
(Min Mandatory) / The Bidder shall provide a response to all items contained within the minimum mandatory requirements attachment of the RFP.
Article 1
(SOW) / The Bidder shall provide responses for all sections of Article 1, Statement of Work, which should be clearly identified using the same format as the RFP, with the appropriate headings.
Attachment A
(Cost Tables) / The Bidder shall provide a statement stating that the cost proposal is included in Pricing Binder.
Attachment B
(Staff Doc’s) / The Bidder shall provide the required Contractor Staff Documents.
Attachment C
(Deliverables) / The Bidder shall provide a reference Article 1 Section 1.104 (XI) (5) (d). Please note the table provided in this section is not an all-inclusive list. Bidders must submit additional deliverables as applicable, and shall identify them in their response to Article 1, or In Attachment C.
Attachment D
(Preliminary Plan) / The Bidder shall provide a Preliminary Project Plan.

(Technical Overview) / The Bidder shall provide a Technical Overview not to exceed 10 pages.

(Security Plan) / The Bidder shall complete and submit a completed DIT-0170 Security Plan along with their proposal.

(Optional) / The Bidder shall provide responses to the Requirements as instructed in Attachment I1or I2 dependent on the submission of a proposal to provide one or both options identified as the Confidential Informant Database and Tip Tool. NOTE: To be awarded these options the vendor must have been selected to provide the primary MCIS solution.
Article 2 / (Bidder must provide written acknowledgement and stated agreement with each term and condition included within Article 2, with particular attention to sections 2.010, 2.030, 2.040, 2.090, 2.110, 2.150, 2.180, 2.190, 2.210, 2.260, 2.270, 2.290, 2.305, 2.307, and 2.308.)
Provide a Statement that a Certificate of Insurance will be provided as a condition of award has been included (referenced in Section 2.180).
Acknowledgment and/or concurrence with each term and condition listed in Article 2 of the RFP/ITB document, has been provided within your proposal, with any comments or issues clearly identified.
Article 3 / Complete all items contained in Article 3, Certifications and Representations, initialing each paragraph requiring an initialed response, acknowledging each certification & representation, and providing all required information.
