Policy

Mental Health Care Foundation, Inc. (MHCF) is a Florida not for profit corporation exempt from federal tax under section 501(a) of the Internal Revenue Code as an organization described under section 501(c)(3) of the Code. The Foundation is organized for the tax-exempt purpose of supporting Mental Health Care, Inc. (MHC or the Agency). Therefore, a fundraising event hosted by a member of the community, an individual, group or organization (the "Event Sponsor") on behalf of the Foundation should uphold the mission, vision and values of the Foundation and the Agency. All funds contributed to the Foundation from the event shall be used in support of programs supported through the Foundation’s mission set forth below. In order to schedule and support appropriate activities through communication, visibility and recognition, the Foundation will evaluate fundraising events to ensure that the criteria set forth below are met and to ensure the fundraising event will be financially self-sustaining. Any individual, group, committee, or other organization wishing to host a fundraising event for the benefit of the Foundation must set forth a written description of the proposed fundraising event on the last page of this agreement (the "Sponsored Event Application"). The completed Sponsored Event Application must be signed by a representative of the sponsoring organization and submitted to the MHCF Executive Director. Once approved by the Foundation Executive Director, the Sponsored Event Application will be forwarded to the appropriate board committee for review and upon approval, will be signed by an authorized representative of the Foundation.

Mission

The Mental Health Care Foundation was created to support and be the fundraising arm for Mental Health Care, Inc. and its programs. The Foundation goal remains constant—to ensure each and every individual receives the finest mental health care available and achieves his or her maximum potential regardless of their ability to pay.

Policies, Procedures and Terms of a Fundraising Event.

Please read the following policies, procedures and terms and acknowledge your understanding of and agreement to them by signing the Sponsored Event Application.

  1. All sponsorship fundraising events must be approved and sanctioned in writing by the Foundation.
  2. All Event Sponsors must be pre-approved by the Foundation to ensure there is no conflict with other key event sponsors or donors currently supporting the Agency.
  3. Sponsorship event dates must not conflict with other events sponsored by the Foundation.
  4. Sponsorship events must be financially self-sustaining without contribution by, or financial risk to, the Foundation. The Event Sponsor agrees that the funds received from the event shall be used for legitimate expenses of the event and further agrees to have a defined plan of action whereby expenses shall not exceed contributions. The Event Sponsor further agrees to fully account in writing to the Foundation the intended distribution of proceeds.
  5. The Event Sponsor should have an image that does not compromise the Foundation's or MHC’s image or conflict with their mission, vision or values.
  6. The Foundation does not sell or endorse products or services provided by other organizations. Therefore, fundraising events that are sales driven or imply endorsement or promotion of a single product or service will not be approved by the Foundation.
  7. All written advertisements, invitations, flyers, brochures and other such documents used for fundraising events must be reviewed by the Foundation before the Event Sponsor uses the Foundation's or MHC’s name and/or logo.
  8. The Foundation does not release donor, volunteer, employee, physician or other mailing lists for the purpose of additional solicitation of funds by outside groups.
  9. The Foundation does not allow, and the Event Sponsor agrees not to participate in, "roadblocking" (solicitation of funds via vehicles on city streets, intersections, etc.) as a method of fundraising.
  10. The Event Sponsor agrees to take responsibility for obtaining all licenses and insurance needed as is warranted by the type of event being held. The Event Sponsor agrees to abide by the rules set by Florida law and other governing organizations controlling such things as gaming rules and liquor licenses, etc. If the Event Sponsor holds a raffle in conjunction with the event, such an activity requires a special license and approval, which is the responsibility of the Event Sponsor. Proof of such license must be provided to the Foundation.
  11. The Event Sponsor agrees to set forth the direct costs that affect tax deductions on the invitation in accordance with the rules and regulations of the Code. The amount of the charitable donation portion of the ticket price must be clearly identified and differentiated from the “fair market value” of the event. Products or tangible items such as the purchase of raffle tickets, greens fees, and goods are not eligible as tax-deductible charitable donations.
  12. The Event Sponsor agrees not to use funds received from the fundraising event in any way that would jeopardize the tax-exempt status of the Foundation or the Agency. The Event Sponsor agrees to comply with any written request by the Foundation that it cease activities that might jeopardize its or the Agency’s tax-exempt status. The Foundation retains the right, if the Event Sponsor breaches this agreement, or if the event jeopardizes the Foundation’s or the Agency’s legal or tax-exempt status, to terminate this agreement immediately.

The policies, procedures and terms set forth above in the Sponsored Agreement for a Fundraising Event are incorporated into this Sponsored Event Application for designating and publicizing the Mental Health Care Foundation as your charity of choice and recipient of the event proceeds.
