PETITION FOR RESTORATION OF DRIVING PRIVILEGES

Comes the Petitioner, ______________________________________________, and would respectfully show unto the Court:

be a habitual offender within the means of the Motor Vehicle Habitual Offender’s Act as set forth in

T.C.A. 55-10-601 et. seq.

2. The Petitioner would further show that since that time he has not violated any laws, and that he has been a good citizen.
