Registration Rules and Requirements:
- Group Swim Lessons are provided for children 3- 12 yrs. Children ages 6 months- 3 yearsold may be registered for Parent Tot Group Swim Lessons. A parent must participate in the water with their child.
- Enclose payment in full for each week, no exceptions. (A $10.00 service charge will apply for returned checks.)Failure to turn in payment with each registration may mean you do not get the first choice of instructor or time.
- No refunds will be granted once session has started, or for missed lessons. Contact the Recreation Coordinator if you have any questions or concerns.
- Registrations are only being accepted in the Administration Office located at the Clubhouse facility.
- You must be a member in good standing to register your family member or guest.
- MEMBERS are responsible for calling in their sponsored guests.
- Registrations are due the Friday before the session you are enrolling in.
- Registrations will be confirmed by phone or email by the Monday morningof the week each session starts.Your lesson is considered confirmed once you turn in your paperwork unless otherwise notified. We will call if any changes need to be made by the Monday before your child’s lesson. Please do not call Admin; they will not be able to confirm your lessons.
- Members have priority registration times and sessions.Member sponsored guests will be added if there is room available. The sponsoring member must be in attendance with their guest. If the member-child registrant and guest child registrant are taking lessons at the same time and would like to be in the same class with the same instructor, let us know and we will do our best to accommodate the request.
- Please fill out Informed Consent, Release, and Waiver Agreement andRegistration Form,upon initial registration for every swimmer. Once the Informed Consent, Release, and Waiver Agreement has been completed, you will only need to submit the session Registration Form again and check the box that says: "Informed Consent, Release, and Waiver Agreement on file” and turn it in with payment by the Friday before you wish to start. If you sign up for multiple sessions at once, (which is highly recommended) you will only need one form filled out for all the sessions you wish your child or children to attend and payment for each session must be made all at once.You can add multiple children on one Registration and Informed Consent, Release, and Waiver Agreement form providing you are their legal guardian and the medical provider information is the same.
- Please notify the Recreation Coordinator one week in advance of each session registered if you need a special accommodation or lesson adaptation so we can give your child the best swimming experience possible.

Please submit this Registration form to Administration Offices during business hours. Monday-Friday, 9:00 a.m.-5:00 p.m. Eachsession runsTuesday through Friday and includesfour 30-min. lessons. No lessons on Mondays.To limit the number of forms to fill out, once you have a completed Informed Consent, Release and Waiver form, please check the box “on file”, so you only fill out the registration page for additional weeks. You can pay for all weeks up front as well as weekly.Payment is due at time of registration.
