2015 – 2016INSTRUCTIONS FOR COMPLETING THE WEB BASED HOUSEHOLD APPLICATION FOR FREE / REDUCED PRICE MEALS IN SKYWARD FAMILY ACCESS
***If you only have a child/children that will be attending West Carroll Primary and/or West Carroll Middle schools you will NOT need to complete a free/reduced meal application due to the CEP (Community Eligibility Provision) that allows every enrolled student (Primary & Middle only) to receive a breakfast and lunch at NO COST, every day for the 2015-2016 school year. ***
To apply for free or reduced price meals for a student enrolled at West Carroll High School,you will need to complete the application following the instructions below. (You will still include any Primary and/or Middle school child/children on your application but the end result of free, reduced or denied application will only pertain to the West Carroll High school children on the application. You can email Cindy Weems at if you need assistance. A new application must be completed each new school year!You may apply for benefits at any time during the year.
You will access the Skyward login on the school district’s website homepage at . If you have not already been set up to view Family Access in Skyward, you will need to contact Brad Field - Technology Director at for a username and password.
***If you will be completing the enrollment process online for the 2015-2016 school year, the free/reduced meal application will be one of the steps in that process. ***
Once logged into Family Access click on Food Service.Click down arrow by “All Students” and select student name. Click “View Applications” and then “Add Application”. You will then see the Letter to Parents and Instructions for Applying. Please read these and check the box confirming you have read the Instructions for Applying. The next steps will be the Privacy Act Statement, Non-discrimination Statement and the Federal Income Chart.
Follow these steps as you complete your application on line. The program will provide prompts to assist you in providing the required information.
PART 1 – ALL HOUSEHOLD NAMES – Complete One Application Per Household Per School District. You will need to enter names of ALL household members and birthdates. School name and Grade columns are for students only. You will need to check the box if the student is a Foster Child. If any member of your household receives SNAP or TANF benefits, list the person’s name and case number and you will be able to skip to Signature page Part 4. Click “Next” button to continue.
PART 2 – CHILD STATUS – Check the appropriate box if any of the students are homeless, migrant or runaway. Click the “Next” button to continue.
PART 3– GROSS INCOME–Enter the dollar amounts of any income received in the appropriate column. To choose the income frequency, click on the down arrow button next to the dollar amount to choose between (W)=Weekly (B)=Biweekly (T)=Twice a Month (M)=Monthly or (A)=Annual. Once all household membersreceiving income are entered with their income amounts and income frequency selection, click the “Next” button to continue.
PART 4 – SIGNATURE–An adult household member must sign the application. Please note, if Part 3 is completed, the adult signing the form must also list the last four digits of his or her Social Security Number or mark the ‘No Social Security Number’ box. Select the “Click to Sign”button which will bring up the Electronic Signature Agreement. Please read the statement and indicate your acceptance by clicking the “I Agree”button. Click the “Next” button to continue.
PART 5 – ETHNICITY AND RACE – This part is optional. Check appropriate boxes if you choose to. Click the “Next” button to continue.
PART 6 – SHARING INFORMATION – This part is optional. Check appropriate boxes if you choose to. Click the “Next” button to continue.
Please review the completed application and fix any indicated errors before submitting for approval. After reviewing for accuracy, click on the “Submit Application” button.
Your application will be in “pending” status at this point. You will be notified when the application has been approved or denied.
*Free & Reduced applications will be available at District Office and High school offices.It will be available on the school website (under Food Service) if you choose to print it from there. They will also be available at registration in July.
