Instructions for Filling out Form BC 2331

The ICORS database is currently stored on each computer’s hard drive and represents many hours of required documentation. Systematic backups are imperative to prevent the loss of vital project records. This schedule is designed to ensure that any data loss is limited to the absolute minimum.

Instructions for Those Not Able to Perform Backups on Central Network Server Systems:

Users are required to make backups (on days ICORS entries were made) on a daily, weekly, and monthly basis. Label each tape/CD clearly with tape/CD name, contract number, and state tag number (if applicable) found on your computer. On workdays when no data entry is made into ICORS, no backup is required for that day and an entry must be made onto the BC 2331 stating “No ICORS Entries” for that particular day along with the initials of that person making the entry.

When possible store monthly and weekly backups away from the field office and/or away from where the computer is kept. If you are using tapes or
CD-RW, your daily backups should be recycled/reused weekly. The weekly backups can be reused every month, and the monthly backups can be reused every other month. An acceptable alternative backup method to the procedures above is the use of USB flash drives. When flash drives are used, it is recommended that a minimum of 2 flash drives be used on an alternating basis. This form must be kept current and shall be subject to review upon request.

Instructions for Central Network Server System Use:

If the user is able to utilize a central network server system and systematic backups are automatically performed daily, the manual backups described above are not necessary. Instead, correspondence must be attached to a blank BC2331 form indicating this is the methodology used and documentation of their automatic file path (location of where the backup data is stored) must be included with this correspondence. The Resident may be asked to demonstrate retrieval of the data upon request.

“Use of ICORS for Documentation” section of the Documentation Guide.
