Faculty Credential Inventory –Form 1 – Instructions

The Faculty Credential Inventory (FCI) is to be completed by the Department Head

  1. Date: Enter date the Faculty Inventory Form is completed.
  1. Name: Enter the Faculty name.
  1. CWID: Enter the college-wide ID.
  1. College: Check the appropriate college for which the faculty member has been assigned. For joint appointments, select all colleges and apply.
  1. Department: Enter the assigned department. For joint appointments, select all colleges and apply.
  1. Primary Teaching Discipline: Enter the discipline area a faculty member has been assigned to teach at the time the form is completed. Note: A new FCI must be completed anytime the teaching assignment changes.
  1. Degree Earned Information: Enter all information for each degree or professional certificate earned.
  1. Qualification Measure: Provide qualification information for discipline areas or specific courses a faculty member has taught or been assigned to teach at the time the form is completed. Note: A new FCI must be completed anytime the teaching assignment changes.

•Qualification Measure: For each line under the Discipline Prefix or Specific Course column, place a check mark in one column that represents how the faculty member is qualified to teach under a given discipline or specific course. Select only one column per line.

  1. Name: Re-enter the Faculty name on page 2.
  1. CWID: Re-enter the college-wide ID on page 2.
  1. Rank: Check appropriate box to indicate rank.
  1. Series: Check appropriate box to indicate series.
  1. Graduate Faculty Status: Indicate the graduate faculty status at the time the form is completed. This form is not an application for graduate faculty membership. It is intended to reflect an existing status. Graduate faculty membership requires a formal application and approval by the Graduate Council. Note: A new FCI must be completed anytime the graduate faculty status changes.

14 - 16. The following table is to be completed onlyif a faculty member does not hold a terminal degree in the teaching discipline, which is indicated under the Qualification section of the table on page 1.

An entry is required for each discipline prefix and/or course that is qualified with a degree outside the teaching field plus other qualifications.

For undergraduate courses, Tables1416 must be completed.

Signatures: The Department Head and College Dean sign the form after reviewing the credentials, including the official transcripts, indicating the faculty meets SACS credentialing guidelines. The signature from the Dean of Graduate Studies is required to verify the graduate faculty membership/status.

Faculty Credential Inventory Supplemental Qualifications Form 2- Instructions

The supplemental qualification is to be completed only if the faculty member lacks a terminal degree in a teaching discipline.

  1. Date:Enter date the Faculty Inventory Form is completed.
  1. Name: Enter the Faculty name and college-wide ID.
  1. CWID: Enter the college-wide ID.
  1. College: Check the appropriate college for which the faculty member has been assigned. For joint appointments, select all colleges and apply.
  1. Departments: For joint appointments, enter all departments that apply.
  1. Rank:Check box to indicate rank.
  1. Series:Check appropriate box to indicate series.
  1. Education – Graduate Degrees: Indicate degrees held, giving major and institution that awarded the degree. (Official transcripts for all graduate degrees are required.)
  1. List Graduate Hours in the Teaching Discipline for Courses listed in Table 14/15 on the FCI.(official transcripts required)
