Plan Do Study Act

Organization: ____ Dental Program

Date: June 1, 2009

PURPOSE/OBJECTIVE OF CYCLE:

What are we trying to accomplish? Improve the quality of services provided by dental hygienists at the ____Clinic.

How will we know that a change is an improvement? If there is no calculus present following scaling and root planing completed by the dental hygienists.

What change can we make that will result in an improvement? Conduct data quality checks, where each dental hygienist evaluates the other following scaling and root planing procedures.

PLAN: the change, data collection and predict

The Change

·  What are we testing? Removal of calculus following scaling and root planing

·  Who are we testing the change on? Patients receiving dental hygiene services at _____

·  When are we testing? June 4-12, 2009

·  Where are we testing?____ Dental

Predictions

·  What do we expect to happen?

That dental calculus will be absent following scaling and root planing 95% of the time (want to achieve 100%)

Data

·  What data do we need to collect? The number of patients receiving scaling and root planing procedures by the dental hygienists (ADA codes 4341 or 4342), the number of patients checked by another dental hygienist following scaling and root planing, and the number of patients with calculus remaining following scaling and root planing

·  Who will collect the data? The two dental hygienists at St. Regis Dental

·  When will the data be collected? June 2-12, 2009

·  Where will the data be collected? In the hygiene rooms at _____ Dental

DO: Carry out the change/test; collect data and begin analysis

·  What was actually tested

·  What happened?

·  Observations

·  Problems

STUDY: Complete analysis of data, summarize what was learned, and compare the data to predictions.

ACT:

·  What changes should we make before the next test cycle?

·  What will the next test cycle be?

·  Are we ready to implement the change?
