Health Self Assessment (SAF) 2012

BROMLEY SUMMARY

Background

This years Learning Disability Self Assessment was completed in July, and submitted to NHS London. A validation meeting will take place with NHS London in October, where a process of validation will be completed. This will follow by NHS London providing final confirmation of Bromley’s performance. /

The Self Assessment, measures performance against 30 key indicators, under the following 3 standard headings:

  1. Access to Health

  1. People with Complex Needs

  1. Safeguarding Governance, Assurance and Quality

Process for gathering information and evidence

Highly valued contributions were provided by service users,which contribute to an essential part of the Self Assessment process.
Their views were gathered through the LD Partnership Board, Getting Ready Meetings and the Big Health Check Day. These workshops focused on discussing the three standards set within the framework.
Service Users, Families and Carers were encouraged to share their views and experiences when accessing local health services. Feedback highlighted where positive experiences had taken place and in some cases, where the service had not met their needs. Suggestions were made where these services could be improved upon. / /

The process continued by meeting and gathering evidence from multi-agency and multi-disciplinary services, Acute Primary and Secondary Health Care Providers, Health and Social Care Commissioners and Local Voluntary Agencies. Once the information gathering process had been completed, Bromleyagreed their predicted scoring against each of the indicators. This was identified by using the nationally agreed LD SAF Benchmarking Tool.

Please note that these predicted scorings may change following the final validation meeting held by NHS London.

The three indicators that have been predicted as scoring REDare:

  • The number of people on the QOF learning disability registers, do not reflect local prevalence data

  • There is no CCG wide referral flagging system used for ensuring that learning disability status is identified to enable ‘reasonable adjustments’ to be made when individuals need to access services

  • South London Healthcare NHS Trust, Acute Hospital Providers (PRUH) have submitted their scoring for this year’s SAF which indicates that they are not performing at the required level of one of the criteria’s set out in the NHS Operating Framework

Feedback and Monitoring

Following this, data and evidence was provided to members of the Learning Disability Executive Group and submitted to NHS London for validation.

A summary report will be provided to the LD Partnership Board Co-Chairs and presented to the Good Health Sub Group.

The Cluster Gap Analysis schedule of the predicted performance has been presented to the Cluster Management Board. This indicates that out of the six boroughs within the cluster, Bromley is favourable to scoring higher ‘Green’ ratings, than other boroughs.

Please note, that these are only predicted scorings.

This predicted evidence, indicates that there could be an overall improvement to Bromley’s scoring to last year. The Red scoring indicators will be the main focus of this year’s action plan, as well as, continuing to make further improvements in the ‘Amber’ scoringcategories. The action plan will be completed once the final validation scoring has been confirmed by NHS London.

The final outcome of Bromley’s performance and the Action Plan will be presented to the LD Partnership and Good Health Sub Group in December. The Good Health Sub Group will be responsible for monitoring the progress of the Action Plan in the forthcoming months.
