3rd ANNUAL CHOCKSETT & PTO FLOOR HOCKEY TOURNAMENT!
To help raise funds for the PTO, Mr. Pageau and Mr. Topo have organized a Floor Hockey Tournament. This tournament is open to all grade levels and will be held after school from November 16th to November 20th.
To participate in the tournament, all players must complete the parent consent form below and return it to school with cash or a check for $5 made payable to WRSD.
There will be two leagues. One is for the 5th & 6th graders and another for the 7th & 8th graders. You may create your own team of up to 8 players. Each team must submit one Team Sign-Up Form. This form must include the first and last name of each player and your team name. The Team Sign-Up form can be printed online or obtained in the Cafeteria. You are allowed to create teams that are a mix of 5th and 6th graders, or mix of 7th and 8th.
If you would like to play but do not have enough players for a team, use the Individual Sign-Up Form in the cafeteria. Mr. Pageau and Mr. Topo will then place you on a team.
Students who are not a part of the tournament may come to watch the games and cheer on their friends. There is an entry fee of $1 for students and $2 for adults.
Please return your permission and team forms into your homeroom teacher by Thursday, November 12th.

Monday, November 16th – Grades 5 & 6
Tuesday, November 17th – Grades 7 & 8
Wednesday, November 18th – Grade 5 & 6
Thursday, November 19th – Grades 7 & 8
Friday, November 20th – Championship Day

RULES
● All players must be in good academic standing.
● Team names must be school appropriate and may be changed by Mr. Pageau or Mr. Topo.
● Each team may select up to 8 players with no fewer than 5 players.
● You may have a mixed 5th and 6th grade team or 7th and 8th grade team. Your team can be all boys, all girls, or a mix of both.
● Mr. Pageau and Mr. Topo may assign students signing up as individuals to any team in their league with fewer than 8 players.
● Players MUST listen to the referees at all times to ensure safety. Failure to comply will result in dismissal from the tournament. No slap shots!
● Players must only use hockey sticks and goalie gear provided by Chocksett.
● Mr. Pageau and Mr. Topo may assign students signing up as individuals to any team with fewer than 8 players.
