2. JOB PURPOSE

To provide high quality comprehensive nursing service to practice population in line with local and national guidelines, to meet health care needs.
Undertake high level and quality nursing care in evaluation and follow up undertaking audit to identify areas of priority and to meet all these health care needs.
Provide evidence based nursing care appropriate to patient's needs.
To share workload with Practice Nurse, making best use of clinical skills enabling diversity.
To assess patients, care plans, provide advice, to maintain patient records.
3. DIMENSIONS
Responsible for providing service to the practice population of GP practice.
·  Number of General Practitioners:- 4 partners, 1 salaried GP and one GPST
·  Number of patients registered with practice:- 5100 (approximately)
·  Number of Practice Nurses:- 1
·  Number of Treatment Room Nurses – 1

5. ROLE OF DEPARTMENT

Provision of comprehensive primary health care for patients including assessment, treatment, preventative care, screening, patient education, and vaccines.

6. KEY RESULT AREAS

1. To provide assessment, screening and treatment services which are relevant to patients and their
families with emphasis on improving diet and exercise routines whilst actively discouraging tobacco
smoking and alcohol abuse through health promotion clinics. Referral to GP and other agencies as
Practice policy dictates.
2.  Assist the Practice Nurse in organising Chronic Disease Management programmes in the Practice including Asthma, Diabetes, COPD, CHD and Thyroid review. The post holder will assess risk factors leading to ill health and advise and assist in suitable interventions for these patients.
3.  Undertake clinical audit in relation to patient care and nursing practice which will help to attain and demonstrate the high standard of clinical practice, and support continuous quality improvement.
4.  Contribute to the development of clinical protocols and guidelines for use in the practice, by making suggestions for service development and improvement and by commenting on changes proposed by others.
5.  To participate in the administrative and professional responsibilities of the practice team and to assist in the formulation of practice philosophy, strategy and policy.
6.  Assist Practice nurse in ensuring the practice medical equipment maintenance programme is up to date and that all equipment is safe and that stocks are maintained.
7.  Responsible for maintaining own professional development and revalidation by attending courses and study days as arranged with the GPs, and completing agreed mandatory practice training and induction.
8.  The nurse will work in accordance with the NMC Code of Professional Conduct

7a. EQUIPMENT AND MACHINERY

·  Routine checking and infection control and sterilisation management of instruments used for minor surgery.
·  Checking of resuscitation equipment in Practice Nurses absence.
·  Cleaning and checking of nebulisers.
·  Responsible for operation and checking of ECG Machine, Spirometer, 24 Hour BP Monitor, Pulse Oximeter
·  Responsible for safe use of personal protective equipment when undertaking procedures or working with bodily fluids.
·  Responsible for ensuring local and national policies for safe systems of work are followed at all times.
7b. SYSTEMS
·  Responsible for the accurate recording of patient information (demographic information, test results, and clinical observations) on local IT systems (i.e. EMIS).
·  Responsible for coding of clinical conditions and health assessments undertaken as part of daily work, as agreed with GPs and Practice Manager.
·  Assist in the coding of recall systems to monitor patients with chronic disease.
·  Ensure that audit systems are in place to monitor own areas of work.

8. ASSIGNMENT AND REVIEW OF WORK

·  As an independent Practitioner, workload is generated by the demands of the GP and Practice population and the post holder is expected to prioritise accordingly.
·  Review of work is ongoing with specific time allocated for discussion at Practice meetings.
·  Annual appraisal takes place where learning objectives are reviewed and set for forthcoming year, and Nurse is responsible for developing, PDP personal development plan and revalidation.
9. DECISIONS AND JUDGEMENTS
The post holder will work within national guidelines and locally agreed clinical protocols.
The post holder will:
·  Advise and assist in suitable clinical interventions.
·  Report Child Protection concerns in line with national and local guidelines
·  Provide onward referral to other services and agencies

10. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB

·  To work autonomously providing evidence based nursing care appropriate to the patient's needs to establish new service provision to meet the demands of the Practice.
·  To prioritise a varied and developing workload on a daily basis to meet the needs of the Practice Population.
·  Maintaining skills in complex multiple clinical areas
11. COMMUNICATIONS AND RELATIONSHIPS
The post holder will often be required to convey sensitive information to patients regarding their clinical condition, and often this will consist of multiple components. They will require to use negotiation and motivational skills when providing, for example, lifestyle advice. There will often be barriers to communication, for example with elderly or very young patients, those with a learning disability or physical impairment, or where English in not their first language.
The post holder must be able to reassure patients when undergoing procedures, and put them at ease.
The post holder will be required to explain procedures to patients to ensure their consent and cooperation.
The nurse will regularly communicate with various people through face-to-face contact, electronically and via the telephone.

12. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOB
Physical Skills:
Venepuncture injections, ear syringing, cervical cytology, subcutaneous injections, suture removal/insertion. Standard key board skills for clinical data entry and report writing.

Mental Demands:
Concentration required when checking documents/patients notes, following chronic disease management protocols, injections, scheduling visits, attending care conferences.
Emotional Demands:
Emotionally demanding elements of the post include dealing with distressed patients, or patient relatives. Some patients may be confrontational. Post holder maybe required to impart unwelcome news to patients. Post holder may, through exposure to clinical records, encounter unpleasant information such as reports of child abuse or neglect.

13. KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE JOB

Registered RGN
Essential Experience and training in:
·  Wound care/ dressings
·  ECG
·  Ear syringing
·  Phlebotomy
·  Immunisation
·  Stitch removal
Desirable experience and training in:
·  Cervical cytology and sexual health
·  Spirometry
·  Travel advice
·  Chronic disease management
Knowledge of relevant IT databases, including EMIS and DocMan.
