Dear Parents of Georgetown ISD Athletes, Band Members, Cheerleaders, Color Guard Members, and Dance Team Members,

In order to prepare for the 2016-2017 school year, there are several required forms and procedures that need your attention. These requirements apply to all incoming 7th- 12th grade students. The following items will ALL be required to clear your child for participation:

** Before you begin please have your athlete’s school ID number available as it is required on each form **

A STUDENT WILL NOT BE CONSIDERED CLEARED FOR ANY ACTIVITIES (INCLUDING TRYOUTS) UNTIL THE FOLLOWING 3 ITEMS HAVE BEEN SUBMITTED:

1. ONLINE FORM submitted electronically. The online form may be found at http://georgetownisd.rankonesport.com under the “electronic participation forms” tab. It is titled “GISD Online Physical Athletic Paperwork”.

(Submit Physical/Medical History Form and Insurance Payment Together)

2. PHYSICAL/MEDICAL HISTORY FORM (Physical dated after May 1, 2016) submitted in person to designated athletic trainer, coach, director, or sponsor - see instructions tab on RankOneSport website for dates and times. The physical/medical history form may be downloaded at http://georgetownisd.rankonesport.com under the “download and print” tab. It is only one sheet titled “Pre-Participation and Physical Evaluation”. The medical history must be filled out and signed by the student and parent. The physical section must be filled out by the appropriate medical provider as indicated on the physical form. GISD requires an annual physical exam.

3. INSURANCE PAYMENT ($10-Football, $5-All Other Activities) submitted in person to designated athletic trainer, coach, director, or sponsor - see instructions tab on RankOneSport website for dates and times. All students participating in athletics (including try-outs) in the Georgetown ISD must submit payment to purchase this insurance policy.

Please see the table below for information on where and when to turn in the UIL physical & medical history form, and catastrophic insurance. Hours will vary due to practices, games, and in-service. It is VERY IMPORTANT to call before coming by the Athletic Training Room.

May 1, 2016 - May 27, 2016 : EVHS Field House Training Room or Middle School Coordinators
After August 1, 2016: EVHS Field House Training Room Middle School Athletic Coordinator when school resumes
