2. Application for obtaining tender documents, must reach this office three days in advance, before opening date of each tender supported by the following documents:-

a. Photocopy of CNIC of MD of the firm

b. Financial Capability/Bank Statement last 01 year

c. NTN certificate

d. Sales Tax Registration Certificate

e. Treasury Challan of PKR200.00, creditable to (Major Head C02501-20, Main Head-12,Sub Head-A, Miscellaneous (Code Head 1/845/30), in favour of CMA (DP) Rawalpindi.

f. Source of supply / OEM of subject equipment Certificate on a judicial paper worth PKR100.00, duly attested by the Oath Commissioner.

g. Certificate

“It is certified that our firm is neither defaulter, nor has been blacklisted by any Government Organization directly or indirectly.” On a judicial paper worth PKR 100.00 duly attested by Oath Commissioner.

3. Detailed terms and conditions are mentioned in the tender documents.

4. Bids of only those bidders shall be entertained, in whose name the tender documents have been obtained, directly or through a local agent.

5. Bid Security 5% of the quoted value (Maximum PKR1.0 Million) through a bank draft/pay order in favour of CMA (DP) Rawalpindi, is to be attached with the offer as Earnest Money.

6. The offer shall remain valid for 120 days from the opening of the commercial offer.

7. Performance Guarantee. 10% of the contract value, shall be provided by the successful bidder, within 30 days after signing of the contract.

8. The Bid: Tech offer, Earnest Money Draft and Commercial Offer, to be submitted in separate envelopes, and indicated on each envelope, about content boldly i.e Technical Offer, Earnest Money Draft and Commercial Offer. All these 3 (three) x envelopes, to be sealed in one large sized envelope and market with Tender Enquiry number.

9. The tender documents can be submitted/ deposited till 0830 hours on the opening date of tender, in the tender room reception of DGP (Army).

10. The tenders shall be opened in the Tender Room of DGP (Army), in the presence of firm representatives.

11. The competent authority, reserves the right of acceptance/rejection of tender and change in the quantity of items.

12. Alternative offer shall not be accepted.

a. Sole agent certificate must be provided duly attested by Embassy.

b. FDA of USA or international standard for Medical devices marked (ISO 13485), JIS, EMSD.

7. Must be compatible with HMIS used in AFIC&NIHD and AFIC IT department team will confirm its integration before opening of financial bid.

8. Training.

b. Technical training of biomed staff by OEM trained Engineer.

c. Software loading and analysis of each and every protocol.
