Target Audience
Referrers to the Haematology service
Purpose
· To assist health professionals with the referral process.
· To ensure all relevant clinical and administrative information accompanies referrals to ensure timely referral processing.
where possible referrals should be made directly to the appropriate tumour stream as follows:
· Allograft Bone Marrow Transplant: Professor David Ritchie
· Acute Leukaemia MDS: Dr Ashish Bajel
· Myeloma/Autografts: Associate Professor Simon Harrison
· Aggressive lymphoma: Dr Michael Dickinson
· Indolent lymphoma: Dr Con Tam
· Myeloproliferative neoplasms: Dr Kate Burbury
· Non-malignant haematology: Professor Jeff Szer
· Late effects: Dr Kylie Mason
General haematology referrals should be directed to Professor John Seymour
Haematology Service: Referral requirements
· A completed Peter Mac referral form, addressed to a named clinician – referral form and further information available at www.petermac.org/referrals
· Any hard copies of scans.
· Any bone marrow biopsy results
· Any biopsies
· Specific Bloods:
o Lymphoma: FBE, U&E, LDH, Electrophoresis, Calcium, Urate
o Myeloma: FBE, U&E, Calcium, ESR, Beta 2 microglobulin, Magnesium, Electrophoresis, SFLC
o Leukaemia: FBE, U&E, LDH, Coagulation, Urate
o Skin Lymphoma: FBE, U&E, Skin biopsies, LDH, LFT’s
o Bleeding Disorders: FBE, Coagulation
HAEMATOLOGY SERVICE CLINICS RUN AT PARKVILLE EACH MONDAY, TUESDAY, WEDNESDAY AND THURSDAY AFTERNOONS, AND WEDNESDAY AND FRIDAY MORNING
· Urgency and clinic allocation are determined by referral content; all referrals will be triaged within 48 hours.
· All new patient referrals are assessed by multi-disciplinary teams.
· Referral forms must include all requested information and patients should bring, wherever possible, copies of any diagnostic imaging, preferably on CD, to their first appointment.

Requesting a second opinion
Please clearly state on the referral form that you are requesting a second opinion and provide a comprehensive summary of all previous treatment including copies of correspondence from previous treating specialists, operation notes, scan reports etc.
