Position Overview

The PM Float Direct Support Staff supports individuals with disabilities in community-based residential homes. Direct Support Staff assist individuals with daily living skills development, health and wellness management, medication administration, behavioral and communication development, personal cares and community involvement. Quality support services will be delivered with dignity and respect for each individual’s unique needs. Direct Support Staff advocate for individuals to be fully integrated into community life. The PM Float will be responsible for filling open and/or temporary PM positions as needed.

Essential Job Functions

1.  Read, understand and implement the individuals Life Area Plan as written for each person.

·  Assists each person with his or her personal cares and daily routine.

·  Shows respect and dignity while assisting each person.

·  Provides choices and promotes independence at home and in the community.

·  Supports people in establishing and maintaining relationships with friends, family, and community connections.

·  Assist and accompanies each person in achieving their personal goals and meeting their needs and desires by participating in leisure and recreational activities and encouraging community involvement such as Community Education classes, Parks and Recreation activities, Special Olympics, shopping, spiritual activities, etc.

·  Clearly and accurately documents information related to each person’s daily program plan.

2.  Read, understand and implement the Individual Abuse Prevention Plan, Vulnerability Summary and Coordinated Service and Support Plan (CSSP) Addendum as written for each person.

·  Demonstrates ability to support each person’s dignity of risk while following through with specific plan/s to reduce vulnerability in identified areas.

·  Clearly and accurately documents incidents related to the person’s vulnerabilities and communicates information as outlined in the person’s individualized plan.

3.  Read, understand and implement the Health Plans as written for each person.

·  Administers medications and treatments as directed.

·  Is able to locate and use medical information established by the program nurse.

·  Clearly and accurately documents changes in the person’s condition and communicates this information to the nurse and coordinator.

4.  Completes work duties as assigned.

·  Provides a clean and safe environment for each person.

·  Prepares and provides balanced meals according to the basic food groups.

·  Clearly and accurately documents and reports maintenance/ household needs and other questions or concerns to the appropriate supervisor.

·  Arrives on time and works his/her scheduled shift.

·  Maintains staff coverage until relief staff is on duty or until a charge person has given approval to leave.

·  Accepts and offers guidance and instruction to/from co-workers in a helpful manner.

·  Attends mandatory Staff Development, monthly House Staff Meetings and other trainings deemed necessary by the supervisor.

·  May be asked to work direct support at another program within Hiawatha Homes to assist with vacant/ open shifts.

·  Other duties as assigned by House Coordinator

5.  Float Duties

·  Report to the PM Float House Coordinator

·  Will be responsible to fulfill open PM Shifts (available 2:00pm-10:00pm) or temporary work assignments as scheduled by the House Coordinator.

·  Will be responsible to work every other weekend

·  Will complete 2-4 training shifts at each program site and be responsible to be knowledgeable and trained on the individuals personal, program and medical needs. Before working unsupervised in a new program, the DSS Float will be competent to complete the necessary job duties; additional training will be provided as needed.

·  Will be responsible to give their House Coordinator at least two weeks’ notice for any Time Off Request.

·  Will be scheduled for a minimum of 60 hours per pay period. If 60 hours per pay period are not fulfilled you will be compensated at your regular wage for the difference in those hours. This position is considered full-time and will be guaranteed enough hours to maintain benefit eligibility.

·  Will be expected to fulfill any PM shift that is scheduled by their House Coordinator.

·  Will not be required to work overtime, double shifts or shifts longer than 16 hours, unless agreed upon by both Float Staff and House Coordinator.

·  May be reassigned at the last minute due to program/ schedule changes. At this time it will be the responsibility of the Float Staff to contact the House Coordinator or the on-call supervisor. Every effort will be made to fill a shift with 24 hour notice but situations may arise and a 24 hour notice may not be feasible.

Minimum Job Requirements

·  Must be at least 18 years of age

·  Ability to speak, write, and comprehend the English language

·  A valid driver’s license, satisfactory motor vehicle record is required for morning and evening shifts and successful completion of Hiawatha Homes’ driving test.

·  Upon hire, candidates must provide proof of employment eligibility and proof of being free from tuberculosis.

·  All candidates must successfully pass a State of Minnesota background check before working unsupervised.

·  Ability to work with and support both male and females

·  Successful completion of Hiawatha Homes Medication Administration class

·  No current corrective action above a written warning.

·  High School Diploma or GED (General Education Diploma) Preferred

·  Frequent pushing, pulling, grasping, reaching below shoulder level, and lifting/carrying up to 35 lbs. unassisted may be required

·  Occasional bending, stooping, squatting, kneeling, climbing, crawling, walking on uneven ground, reaching above shoulder level, and lifting/carrying in excess of 35 lbs. unassisted may be required

·  This position requires spending the majority of the work day standing or walking

·  This position requires working 75-90 percent indoors and 10-25 percent outdoors

·  This position may require work with household cleaning solutions, electric wheelchair batteries, and refueling vehicles

·  This position may require work with moderate noise and/or pets
