Determination of Growth for School nurses based on critical SISP role in educating students

The following rubric can be used to determine student growth when addressing chronic health care issues. It may include; asthma, diabetes, life threatening allergies, anxiety, headaches, etc.

  1. Direct Measure – provide a brief 5-question survey to be conducted with students entering the health office, in September and again in May.
  2. Directions to administer 5-question survey. For random selection of students identify a 2-week period (10 days) to offer the survey.
  3. Randomly select students by:

Day 1 offer to the first 5 students who enter the health office from 8:30a – 11:30a

Day 2 offer to the first 5 students who enter the health office from 11:30a – 2:30p

Day 3 offer to the first 5 students who enter the health office from 8:30a – 11:30a

Day 4 offer to the first 5 students who enter the health office from 11:30a – 2:30p

Day 5 offer to the first 5 students who enter the health office from 8:30a – 11:30a

And so on for the next 5 days

  1. Survey will be completed with student in designated amount of time.
  2. Answer key will be created to score the survey, comparison will be done between September and May results.
