About the desktop review

Once you have successfully completed the PQASSO self-assessment process you will be ready to move on to the first stage of the PQASSO Quality Mark assessment – the desktop review. It is important that you provide all the documents required.

At the desktop review stage, the assessors will assess your key organisational documents. The purpose of the desktop review is to help the assessors determine whether your organisation is ready for the site visit. It also helps them prepare for the site visit. All information provided to the assessors will remain secure and confidential.

It is important that you provide all the desktop review documents requested by the assessors by the deadline given. If you do not provide all documents, or do not provide them on time, the assessment may be cancelled and you could incur a cancellation charge. (Details of cancellation charges are given in the Assessment General Terms and Conditions.)

If the desktop review reveals that your organisation is not ready for the site visit, the assessment may have to be cancelled and you will incur a cancellation charge. (Details of cancellation charges are given in the Assessment General Terms and Conditions.)

How to complete the desktop review list

You will need to provide the following documents for the first stage of your organisation’s PQASSO Quality Mark assessment – the desktop review.

Organisations being assessed at level 2 of PQASSO are required to produce the desktop review documents for level 1, as well as all the documents for level 2.

You will need to gather a set of documents for each assessor. You must indicate for each document to which evidence requirement on the desktop review list the document corresponds. Mark the documents clearly with the reference – for example, L1D1.

A set of these documents should be sent by post or email, as requested, to each assessor, together with a copy of the Desktop Review List. This should be marked ‘private and confidential’.

Note that the sources of evidence required do not all have to be separate documents. One document may cover more than one of the requirements, in which case please label the document with each reference.

L1D1 / The completed Assessment Workbook – Level 1
L1D2 / The current version of the governing document
L1D3 / The mission statement, and statement of aims and intended outcomes for defined user groups
L1D4 / The organisation-wide Operational Plan for the current year as approved by the Board, with targets for the most important outputs
L1D5 / The most recent report to the Board on progress against the Operational Plan (L1D4)
L1D6 / A sample from your PQASSO self-assessment and a sample of a resulting action plan
L1D7 / A list of current Board members and officers (e.g. Chair, Vice-Chair, Treasurer), including start dates and induction dates
L1D8 / The current induction Policy/Procedure for new Board members, and a list of documents provided to trustees at induction
L1D9 / The agenda and minutes of the last 4 Board meetings and last 2 Annual General Meetings, plus all documents tabled at each meeting
L1D10 / The most recent Annual Report and Accounts - if not yet published, then the latest draft
L1D11 / The organisation-wide budget for the current financial year as approved by the Board, and the most recent financial report to the Board against this budget
L1D12 / Information for users about your organisation and the services it provides, including any promotional leaflets or flyers
L1D13 / An example of how user feedback is collected
L1D14 / An example of a staff contract and written terms and conditions of employment
L1D15 / Details of numbers of staff and volunteers and job/role titles, including start dates
L1D16 / Information showing your organisation’s line management structure and who is accountable to whom, e.g. organisational chart
L1D17 / A list of all financial procedures currently in place
L1D18 / An example of a collated monitoring or evaluation report demonstrating achievement of outputs, outcomes and user satisfaction
