  1. BRIEF RESUME OF INTENDED WORK :

6.1NEED FOR THE STUDY

Kāmala is one of the major causes of mortality and morbidity in developing countries. Kāmala has been observed to be shifting from sub clinical infection to rapidly producing chronic liver disease1. Its prevalence is more in India, as liver plays a central role in maintaining metabolic homeostasis and toxicity. So it is essential to study diseases and disordered metabolism. Dysfunction of Liver not only affects the metabolism but also the circulation and Haemopoetic activities of the body2.

Ayurveda acharyas explained Kāmala and its bhedas as swatantra and paratantra vyadhi to pandu roga and upadrava of other disease. It is commonly caused due to mithya ahara and vihara leading to dooshana of asthayi dhatus rasa and raktha all together3. This dushti of rasa and raktavaha srotas jointly produces Kāmala roga by affecting the raktavaha srotomoola yakruth and pleeha. Yakrut in particular helps in pachana kriya and production of pitta, which it secretes into grahani.

In this aspect there are so many medicines in use. Among them one is folklore medicine (Ingudi + Ela + Khanda Sarkara). The persons who are advicing this medicine is unaware about the mode of action of the drug. Present study is undertaken to evaluate the medicinal efficacy and mode of action on Kāmala disease.

6.3OBJECTIVES OF THE STUDY

Management of Kāmala by Anubhoota yoga as shamana aushada.

  1. MATERIALS AND METHODS
  2. Source of the data :

Patients attending the OPD / IPD of Ashwini Ayurvedic Medical College Hospital, Davangere and also from special camps conducted in Kottur, (Bellary district) will be selected for the study.

7.2.Method of Collection of data:

Patients presenting the classical symptoms of Kāmala will be selected for the study like Haridra twak netra, mootra, nakha, anana, daurabalya, aruchi, chardi, hrillasa.

INCLUSION CRITERIA:

1.Patients from age group 20–50 years irrespective of gender, occupation, and educational, economical and social status will be selected.

2.Bilirubin level 1 – 5mg/dl, 5 – 15mg/dl, more than 15mg/dl.

EXCLUSION CRITERIA:

1.Patients with complication of this disease will be excluded like Haleemaka and Kumba Kāmala.

2.Kāmala with other systemic disease, which hinders the course of treatment, will be excluded.

3.Chronic liver diseases, cirrhosis of liver, alcoholic liver diseases etc.

STUDY DESIGN

  • Total number of cases selected for study is 30 and above.
  • Single group study.
  • To assess the severity, the following parameter is adopted.

Parameter mild moderate severe

Total serum bilirubin 1 – 5mg/dl 5 – 15mg/dl>15mg/dl

DRUG FORM AND POSOLOGY

  • Kalka form (Ingudi + Ela + Khanda sarkara) in dosage of 5 gms in the morning and evening on empty stomach. Anupana is ushna jala.

DURATION

  • 14 days
  • 2 follow ups once in 15 days.

SUBJECTIVE PARAMETER

  • Reduction in the symptoms of Kāmala i.e. haridra twak, nakha, nayana, mootra, daurbalya, daha, aruchi, hrillasa.

OBJECTIVE PARAMETER

Reduction in bilirubin level and symptoms.

7.3 DOES THE STUDY REQUIRE ANY INVESTIGATIONS OR INTERVENTIONS TO BE CONDUCTED OF PATIENTS OR ANIMALS ? IF SO PLEASE DESCRIBE BRIEFLY.

  • The study will be human clinical study; no animal experimentation will be done.
  • The study will be done on patients who match inclusive criteria only.

STATISTICAL ANALYSIS

Results will be analysed by using standard statistical test – unpaired (t- test).
