FACULTY SELF EVALUATION FORMAT

2016-2017

Name: ______Date: ______

Please respond to each of the following questions in narrative or outline form or in a list, as you wish. Your responses should be an evaluation of yourself incorporating your goals from the previous year. In each category you should comment upon progress toward, or completion of, goals related to that category. Tenured faculty may elect to comment upon progress toward completion of long-range goals rather than annual goals. This format assumes you will prepare your responses with word processing and insert them below each category. You should complete your evaluation and submit it to your department chair in advance of your annual meeting with your department chair (VPAA if you are the department chair). Return to Academic Affairs electronically to:

  1. Teaching: Evaluate your teaching this year including highlights, innovations, special circumstances in courses (such as first time taught), results of your student course evaluations, and any other assessment evidence you may care to include that demonstrates the achievement of student learning on the appropriate outcomes. You should determine what went well and what didn't go so well and why, as well as what you will do to further improve your effectiveness as a teacher.
  1. Fulfillment of advising and other responsibilities to students: Discuss how you interacted with students outside of your courses this year– advising, mentoring, availability to students, with student groups, involvement in student/faculty research projects, spontaneous interactions, etc
  1. Continued Professional Development: Discuss your professional development activities this year. Where possible, focus on how the activities benefited Morningside College. Note the creation of products of professional development activities from this year (such as publications, papers, creative presentations, performances, etc.) and products under way, but not completed, thisyear.
  1. Fulfillment of obligations to the faculty and the college: Discuss service to the faculty and college this year. Where possible emphasize leadership activities or other significant accomplishments. Service may include such things as service on committees and taskforces, helping with admissions events, visiting with prospective students, etc.
  1. Service to the community, local and beyond: Discuss your community service this year. Where possible, emphasize how the service extends the reach and good reputation of Morningside College and provides positive examples for our students.
  1. Goals for the next year: Give 3-7 goals for the next year and, for each goal, indicate which of the five categories it addresses. (Occasionally, a goal may address more than one category.) It is not necessary that each category be addressed by one or more goals. It is generally best if goals focus on the areas identified in the self-evaluation as most needing improvement, or on innovations that the faculty member desires to implement; that they be specific enough to make it easy to determine when they have been achieved; and that fulfillment of them depend on actions of the faculty member rather than external factors. Tenured faculty may elect to restate their long-range goals, noting and explaining modifications, if any. Tenured faculty members who elect to do this should also indicate what they intend to do during the coming year to progress toward completion of their long-range goals.
