Dementia Learning and Training Questionnaire

This is a short survey on dementia learning and training.

Your reply will be treated in confidence.

  • By training we mean class-room based sessions.
  • By learning we are referring to any activity in which you learn. This might be carrying out a new task at work, through discussion at supervision or through shadowing someone more experienced than you. There are lots of ways we learn at work.

We will be using your response to help us understand the current level of dementia learning and training in Oxfordshire and to direct future training needs.

It should take you no more than 15 minutes to complete this questionnaire.

Completed paper versions should be sent to:

  • Oxfordshire Association of Care Providers, Stables 4, Howbery Park, Benson Lane, Crowmarsh Gifford, Wallingford, OX10 8BA
