IFSP Re-activation - NYEIS Version 4.01

POLICY:

A Municipal Program User Administrator may re-activate a closed Individualized Family Service Plan (IFSP) and/or Service Authorization (SA) in NYEIS.

PURPOSE:

To correct designated inaccuracies in the data system rather than submit a data change request through the Health Commerce System secure file transfer.

GENERAL INFORMATION:

NYEIS Version 4.01 permits designated municipal users in the role of Municipal User Administrator, the ability to re-activate a closed IFSP and/or Service Authorizations in order the correct inaccuracies in the data system. These corrections should be made to accurately reflect services agreed to by the family and EIO/D during the corresponding IFSP period. Any data being corrected must be appropriate and in accordance with EI regulations and policies and accurately reflect the services delivered.

Allowable changes are:

·  Extend an IFSP due to CPSE Eligibility

·  Process Tasks Related to Bilingual Supplemental SAs

·  Correct a Supplemental Evaluation that should have been Authorized in a Different IFSP

·  Correct Information in the “Demographic” Portion of an IFSP

·  Correct a Closed SA in an Active/Approved/Extended IFSP

·  Amend an SA for an Increase in Services

·  Amend an SA for a Decrease in Services

·  Amend the Provider of Record

·  Add an SA for Needed Co-Visits

·  Add an SA for Needed Make-up Visits

·  Add an SA to Reflect Services Authorized

·  Add an SA to Increase Units (Service Coordination)

PROCEDURE:

·  The municipality will designate a Municipal Program User Administrator who will have authorization to re-activate a closed IFSP and/or Service Authorization in NYEIS. This individual holds the “Muni Program User Admin” user role for NYEIS.

·  When there is a need for a correction to an IFSP/SA in NYEIS, a request will be made to the Municipal Program User Administrator.

·  The Municipal Program User Administrator will assess the request to determine whether it is one of the allowable changes permitted NYSDOH, is appropriate and in accordance with EI regulations and policies and accurately reflects the services delivered and agreed to.

·  If it is determined that the request is appropriate, the Municipal Program User Administrator will go to Activate Online in the manage cluster at the top of the IFSP/SA and follow the guidance as prompted on the screens to re-activate the IFSP/SA.

DOCUMENTATION:

·  Changes are recorded in NYEIS.

·  NYEIS provides notifications to affected parties.
